The Colorado Health Foundation hosts an Application Information Webinar for February 15th application deadline

The Colorado Health Foundation is hosting an application information webinar on Friday, January 10, 2014 at 11:00am for prospective grant applicants.  The webinar gives representatives from nonprofits that work in health care, health coverage and healthy living a chance to learn about the Foundation’s grantmaking program.  Register now.

State Health and HealthTeamWorks release pregnancy-related depress guideline

HealthTeamWorks and the Colorado Department of Public Health and Environment (CDPHE) are releasing their Pregnancy-Related Depression Guideline. This new resource supplements the widely used Adult Depression Guideline previously developed by HealthTeamWorks. The supplement covers risk factors, cross-cultural considerations and suggested approaches. HealthTeamWorks and CDPHE will continue their collaboration in 2013-14 to implement the PRD supplemental guideline in diverse settings around Colorado. New Resources from HCPF Online Now

"Next Enrollment Metrics Release:  Connect for Health Colorado and Colorado Medicaid will release joint enrollment metrics on January 2, 2014.  This update will include metrics for enrollment through December 31, 2013.

Accountable Care Collaborative Reaches Enrollment Milestone:  The Department recently announced it has enrolled its 400,000th member into the Accountable Care Collaborative (ACC) program.  To view the Department's press release click here and for more information about the ACC click here.

PEAK Instructions Available Online:  The Department has added an additional online resource.  Instructions for printing a Medicaid card, making CHP+ and Medicaid buy-in payments, and completing a redetermination online are available at Colorado.gov/HCPF/ACAResources under the PEAK Resources heading

Resources for Agents & Brokers:  The Department has developed a new set of resources for Agents and Brokers including a webinar and the corresponding slides on PEAK and how to increase the likelihood an application may receive a real time determination.  You can view these new resources here.

Medicaid Benefits Starting Jan. 2014:  Important information has been released about the Medicaid benefits for 2014.  This fact sheet provides an overview of the benefits for current and newly eligible Medicaid clients beginning Janay 2014.

Cancer Survivorship E-Learning Series for Primary Care Providers

The Cancer Survivorship E-Learning Series for Primary Care Providers is a program of the National Cancer Survivorship Resource Center. The objectives of the 5 module series are to: 1) increase PCP awareness of the ongoing needs of cancer survivors, 2) increase PCP knowledge of how to care for cancer survivors, and 3) increase PCP ability to provide follow-up care for cancer survivors. Each module consists of one or more on-demand webinar presentations by clinicians and experts; interviews with cancer survivors; interactive features such as quizzes and patient/survivor case studies; and access to tools and resources to improve providers' knowledge and competencies related to post-treatment care of cancer survivors.

Heart Disease and Stroke Prevention Guideline Resources for Patients and Providers

“The newest heart disease and stroke prevention guidelines for doctors, released on November 12 by the American Heart Association and the American College of Cardiology urge doctors to help patients avoid heart disease and stroke by prescribing drugs called statins for some, treating obesity as a disease, and providing other resources to help their patients stay healthy.

Should you change your medications? Should you see a doctor for obesity treatment? How do you know if you’re healthy? These are among the questions patients can find answers to on the American Heart Association's website through their Guideline Resource Center.  Check out the What Guidelines Mean to You infographic.

Local Foundations Provide Support to Expand CSPH Partnerships and Outreach

Local Colorado foundations signaled ongoing support for improved community health with the announcement of a new, collaboratively funded grant to the Colorado School of Public Health. The Caring for Colorado Foundation, Colorado Health Foundation, Colorado Trust and Rose Community Foundation collaborated to donate $1.5 million to expand the school of public health's academic-community partnerships.

The grant will provide support to accelerate the school's move to online education for rural health professionals, build community health improvement capacity in response to the Affordable Care Act, and provide a forum for discussion on pressing public issues. The school plans to report on the grant's activities, including highlights of community activities, through the Center for Public Health Practice, publichealthpractice.org.”  [Source:  COPrevent]

Update on Enrollment and Key Dates to Know

The Department of Health Care Policy and Financing issued a joint press release with Connect for Health Colorado on updated enrollment metrics. You can find more information about the Medicaid expansion metrics by viewing our fact sheet.

Key dates for you to know:
  • December 20, 2013 – Date by which Coloradans are encouraged to submit applications for financial assistance for coverage that started January 1, 2014.
  • December 23, 2013 – Deadline to select a private health insurance plan on Connect for Health Colorado that will take effect January 1, 2014.
  • January 1, 2014 – Earliest date coverage begins for newly eligible Medicaid or private health insurance plans purchased through Connect for Health Colorado.
  • January 10, 2014 – Deadline for Connect for Health Colorado customers to make first month’s premium payment for January 1, 2014 coverage.
  • March 31, 2014 – Open enrollment for Connect for Health Colorado ends (Note: Medicaid does not have an open enrollment period).”  [Source:  HCPF]

The Colorado Trust Offers New Funding Opportunity

Donation of Flu Vaccine from Sanofi – Please Order with AmeriCares by December 20th

AmeriCares recently received a donation of flu vaccine from Sanofi-Pasteur, and is making these vaccines available to free clinics across the county.  Product information for Sanofi’s intradermal FluZone can be found here.  Make sure to submit all requests by Friday, December 20th, 2013.  You can expect to receive the vaccine within a few weeks of submitting your requests.

New Framework for School Behavioral Health Services

“The Colorado Legacy Foundation (CLF) recently released the Colorado Framework for School Behavioral Health Services, which includes a best practices blueprint for comprehensive school behavioral health services, a roadmap of critical success elements, and tools and resources to support schools in their implementation of comprehensive behavioral health systems.

The Framework supports the integration of behavioral health supports in schools with local behavioral health systems through a Multi-Tiered System of Supports (MTSS) combined with a System of Care (SOC).  The development of the Framework was guided by a leadership advisory committee comprised of education and behavioral health professionals and was informed by a literature review, interviews and focus groups, and a statewide gaps and barriers analysis.  Click here for more information.”  [Source:  COPrevent]

AAP accepting applications for Tobacco Cessation Clinical Practice Training

The American Academy of Pediatrics Julius B. Richmond Center of Excellence is seeking applications for a training that will educate clinicians about messages, tools and counseling protocols to screen and counsel patients and families about cessation and secondhand smoke exposure during clinical encounters. Clinicians will be taught how to effectively integrate these practices into their existing practice systems. Applicants are encouraged to attend the training in pairs of a pediatrician and another member of the practice team. There will be two separate trainings and applicants will attend one of the two. Coach airfare, transportation between airport and hotel, hotel accommodations (2 nights maximum), and meals included. For more information, read the application. HealthTeamWorks is accepting applications for the 2014 installment of the PCMH Foundations program and Innovations program

HealthTeamWorks is now accepting applications for the 2014 installment of our PCMH Foundations program after receiving a generous grant from the Colorado Health Foundation. The grant, which will provide practice transformation coaching to more than 100 Colorado practices free of cost, supports a program starting in January of 2014.

In an effort to provide support to those practices early on in their transformation while also providing advanced curriculum to practices across Colorado already on their transformation journey, the program will encompass two parts.

A continuation of the current PCMH Foundations program will continue to lay the groundwork for practice transformation of new practices and those still early on in their journey to practice transformation.  A more advanced Innovations program will expand on pre-established concepts and continue support for many practices who are looking to achieve the Triple Aim.

CMS Revises Meaningful Use Timeline: Stage 2 Extended Through 2016

The Centers for Medicare & Medicaid Services on 12/6/13 announced a revised timeline for Meaningful Use Stage 2 and 3. Under the revised timeline, Stage 2 will be extended through 2016 and Stage 3 will begin in 2017 for those providers that have completed at least two years in Stage 2.

According to their announcement the goal of this change is two-fold: first, to allow CMS and ONC to focus efforts on the successful implementation of the enhanced patient engagement, interoperability and health information exchange requirements in Stage 2; and second, to utilize data from Stage 2 participation to inform policy decisions for Stage 3.  Click here to read more.”  [Source:  CCI Voice]

New All Payer Claims Data Provides CO with Expansive Health Care Insights

Hospital readmission rates for Medicaid and commercially insured individuals are now available on the Colorado All Payer Claims Database (APCD) website demonstrating, for the first time, quality of care provided to Coloradans under 65.  Generic prescription medication use and claims from 2012 are also now available on the public site, providing Colorado communities with a more expansive view of cost and health care services being used by their population.  Read the full press release and visit www.cohealthdata.org for more.”  [Source:  CIVHC]

Medicaid Breast and Cervical Cancer Program

Effective Dec. 1, individuals diagnosed with breast or cervical cancer by community providers may be eligible for the Breast and Cervical Cancer Program through Medicaid. Previously, eligibility was limited to clients who were diagnosed by certain providers throughout the state. The Breast and Cervical Cancer Program is a Medicaid program for women who have been diagnosed with breast or cervical cancer. Treatment may also be available for certain breast and cervical cancer conditions that may lead to cancer if not treated. PEAK Enhancements Effective December 6th
The Department of Health Care Policy and Financing has made additional upgrades effective December 6, 2013 to the PEAK application for Medical Assistance programs.  The recent upgrades seek to improve the ability for an applicant to receive a real time determination (immediate approval or denial) when completing a Medical Assistance only application.  Click here to find out more about the most recent upgrades.

New Cholesterol Screening Guidelines Announced

“The American Heart Association and the American College of Cardiology announced a new practice Guideline on the Treatment of Blood Cholesterol to Reduce Atherosclerotic Cardiovascular Risk in Adults as well as a risk assessment tool for providers to use. Three other guidelines were also released: two on screening for and management cardiovascular risk and one on the evaluation and treatment obesity. Colorado Finds Savings from Accountable Care Collaborative

“Visit the Colorado pages of NASHP’s Medical Home Map and State Accountable Care Map to learn more about a recent report on the state’s Accountable Care Collaborative (ACC).  The report estimates $44 million gross reduction ($6 million net reduction) in total cost of care in the initiative’s second year, as well as reduced hospital admissions and readmissions, use of high-cost imaging services, and emergency room utilization, and improved chronic disease management. Launched in 2011 to expand medical homes for Colorado Medicaid enrollees, the ACC program uses a regional model to support practices in providing medical home-caliber care.  NASHP has been working to support Colorado medical homes projects since 2007.”  [Source:  NASHP]

The Connect Campaign will conclude in March 2014

"The Colorado Coalition for the Medically Underserved has been proudly convening the Connect to Coverage, Connect to Care Campaign since April 2011.  As a result of this Campaign’s great work, we had a successful implementation of the limited Medicaid expansion for low-income adults without dependent children and we are on our way to increasing the eligibility levels to 133% for this same population (as well as for parents).

We have decided that the Campaign has accomplished its mission and the time is right for this group to conclude in March 2014.  You can still engage in the ongoing work with Covering Kids and Families, who will continue to focus on eligibility, outreach, and enrollment for kids and parents, along with monitoring and proposing solutions for strong implementation of the ACA in Colorado. They welcome your insights and participation in their work groups or by subscribing to their monthly newsletter.

Coloradans are now able to check the status of their Medical Assistance Application (includes Medicaid and Child Health Plan Plus) by calling 1-800-359-1991.  Customer service representatives are available to provide assistance between 8am and 6pm Monday-Friday.  Applicants will need to provide their Application Tracking Number.

RFP: Basic Human Needs Community Navigator Learning Cluster

The Denver Foundation is requesting applications to participate from nonprofit organizations interested in joining the Basic Human Needs Community Navigator Learning Cluster.  This project is a two-year intensive Peer Learning Cluster among nonprofit and resident community navigators designed to leverage existing assets to develop a system of Basic Human Needs Community Navigators within targeted Metro Denver communities.  The work of this Cluster will focus on connecting low-income immigrant and refugee communities and low-income communities of color to available services.

HCPF New Consumer Resource: How to check your application status

“Coloradans are now able to check the status of their Medical Assistance Application (includes Medicaid and Child Health Plan Plus (CHP+) by calling 1-800-359-1991 between 8:00am and 6:00pm Monday-Friday (closed state holidays).  Customer service representatives will be on hand to assist with identifying the status of a caller’s application, and if needed, obtain the information needed to process their application.  Applicants will need to provide their Application Tracking Number, found on the final screen of the online application when they call.

Federal law requires all applicants seeking financial assistance to help buy insurance through Connect for Health Colorado marketplace be screened for Medicaid first. Once it is determined a person does not qualify for Medicaid, the individual can then apply for financial assistance to buy insurance through the Connect for Health Colorado marketplace. In Colorado, the average processing time for a Medical Assistance application is 16 days.

Who is a candidate for diabetes prevention?

“An estimated 80 million Americans have prediabetes, yet only one in 10 is aware of it.  Few patients take advantage of a prevention program like the Diabetes Prevention Program that can help them avoid type 2 diabetes.  Eligibility for participation in the evidence-based program is open to overweight and obese adults who have a recent blood test scores in the prediabetic range, or who score nine or more points on the CDC’s prediabetes screening test.  Have your patients or clients call 1-800-DIABETES to find Diabetes Prevention Programs in Colorado.”  [Source:  COPrevent]

Survey for Medicaid Clients

"This survey is being conducted to collect input from Medicaid consumers about their preferences for receiving primary care in the Accountable Care Collaborative (ACC) program.  All surveys must be complete by Wednesday, December 4th, 2013 (online survey closes at 5:00pm). Please note that the deadline has been extended from November 29th to December 4th.
This survey is being conducted by a workgroup of the Provider and Community Relations Subcommittee, which is part of the ACC Program Improvement Advisory Committee.  The results will be used to inform discussion about potential policy recommendations to the Department of Health Care Policy and Financing.  The survey may be completed by an adult Medicaid recipient for his or herself and for each of his or her children who receive Medicaid. Connect Campaign wants to hear from you!

ClinicNET continues to participate in the Connect Campaign, a collaboration of stakeholders formed in 2011 to fulfill the promise of coverage and care for newly Medicaid eligible low-income adults without dependent children (AWDC.) Over time, the Campaign’s focus broadened to ensure Medicaid’s ability to cover and care for all of its populations efficiently and effectively.

ADA Offers Free mHealth Program: Living With Type 2 Diabetes

"To guide and support individuals with type 2 diabetes, the American Diabetes Association is offering a free 12-month program called Living With Type 2 Diabetes.  The program provides information and offers a free guidance to help people learn how to manage diabetes at regular intervals throughout the year-long.

The program is available in English and Spanish and participants can choose to receive information online or through the mail.  Participants can also choose to receive additional recipes, tips and reminders on their phone.  People can enroll into this free program by visiting the website, calling 1-800-DIABETES, or texting Type2 to 69866 to learn more about the program in English or Tipo2 to 69866 to learn more about the program in Spanish.

How Well do Healthcare Organizations Serve Low-Income Communities? Comment on Metrics Now Being Developed

"The Center for Health Market Innovations is developing standard definitions for relevant social and financial performance metrics commonly tracked by healthcare organizations serving low-income communities.  The goal is to provide a tool for impact investors, funders, and program implementers to help answer the question of how well these organizations serve the poor.  See more on the metrics development process.

Science Advisory Released: Hypertension Control Protocol

"Improving clinical performance is a top priority for CDC's Division for Heart Disease and Stroke Prevention.  Million Hearts has prioritized improving blood pressure control in the US.  As part of this work, this week, CDC, the American Heart Association, and the American College of Cardiology issued a science advisory recommending that US practices and health systems adopt and use evidence-based hypertension control protocols.

New Safety Net Advisory Council (SNAC) Learning Lab materials available online

"The Colorado Health Institute convened the fifth SNAC Learning Lab of the 2013-2014 season on November 14th.  This session focused on new data sources that provide the consumer perspective.  The Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey measures the patient experience within the Medicaid Accountable Care Collaborative.  The 2013 Colorado Health Access Survey (CHAS) providers a consumer perspective on a range of health issues, including barriers to care.  Thank you to those who attended.

The Colorado Health Institute team has posted materials from the meeting here. A summary of the discussion in the fifth of a new series, Food for Thought: Updates from the Safety Net Advisory Committee (SNAC), will be posted in the next few weeks.

Diabetes’ Impact on Colorado Fact Sheet Released

The Colorado Department of Public Health and Environment released a new fact sheet on diabetes.  See the Diabetes’ Impact on Colorado Fact Sheet. The number of adult Coloradans with diabetes has soared 157% during the past decade, from 4.7% in 2003 to 7.4% in 2012.

Help is available for Coloradans at risk for diabetes.  The Diabetes Prevention Program, now being offered by the YMCA of Metropolitan Denver, the American Diabetes Association and community groups across Colorado can prevent or delay the onset of type 2 diabetes. Social Determinants of Health – Video Released by CCMU

"The Colorado Coalition for the Medically Underserved released a video on November 13th highlighting several factors that interfere with patients’ ability to get health care needs met. Watch the video here, featuring ClinicNET affiliates! These factors, known as social determinants of health, are things like income, education, access to quality food and housing, race, or ethnicity. A patient’s ability to navigate the health care system and get their needs met is defined by these social determinants and their provider’s ability to work with those challenges.

This video aims to increase awareness and empathy among current and future providers, as well as encourage creative thinking to find new solutions to these old problems. We believe that this is key to improving patient outcomes and compliance. We believe that the field of medicine is a science, but the practice of health care is an art that acknowledges the whole of the patient’s life.

AmeriCares is a global health and humanitarian non-profit dedicated to responding to the needs of the health care safety net around the world.  In the US, we work with free clinics, FQHC’s and other safety net organizations to provide medications, vaccines and supplies to low-income, uninsured and underinsured patients.

Update on Medicaid Benefits for Newly Eligible

Beginning January 1, 2014, more Coloradans will be newly eligible for free or low cost coverage through Colorado Medicaid. Currently, Medicaid offers primary care, behavioral health, hospitalization, rehabilitative services, laboratory services, outpatient care, prescription drugs, emergency care, dental care, maternity care and newborn care.  The benefit package for newly eligible Medicaid clients will be largely the same as the current Medicaid benefits, including the new dental benefit and enhanced mental health and substance use disorder benefit.

In accordance with federal law, the newly eligible Medicaid clients will receive preventive and wellness services as defined by the U.S. Preventive Services Task Force. In an effort to align Medicaid benefits, the current Medicaid benefit package will be expanded to include these preventive and wellness services.  Additionally, the newly eligible Medicaid clients will receive habilitative services, which are considered to help individuals maintain skills necessary for daily living.

The benefit package for the newly eligible providing general Medicaid coverage, additional preventive and wellness services and habilitative services must be approved by the Centers for Medicare and Medicaid Services. The Department of Health Care Policy and Financing will continue to update you on the approval process.

New Funding from TGYS Available

The TGYS Program is excited to announce the release of the Request for Application (RFA) for the next three-year funding cycle that begins July 1st, 2014.  TGYS provides funding for community-based programs that target youth and their families with prevention and intervention services in an effort to prevent incidents of youth crime and violence, and child abuse and neglect.

To begin the TGYS application process, visit the TGYS website at: www.colorado.gov/CDHS/TGYS and download the application. The TGYS Program highly recommends that all potential applicants watch the RFA Technical Assistance Webinar, also posted on the TGYS website. This webinar provides information about the TGYS program, outlines the RFA requirements and provides resources and tips for submitting a competitive application.

Applications must be submitted by email and received by 4:00 pm Mountain Time on Wednesday, December 18, 2013. Additional instructions are provided in the RFA document. State Health supports Great American Smokeout with partner communication tools

The Colorado Department of Public Health and Environment has developed Great American Smokeout communications tools to support our many partners calling on Coloradans to join millions of Americans on November 21 as they break free from their addiction to tobacco.

It isn’t easy to quit, but it’s worth it, and there is help available. In addition to CDPHE tools, there are many resources available for Coloradans ready to quit smoking. Smokers and their families can learn more about tobacco at tobaccofreeco.org. Those who are ready to quit can call the Colorado QuitLine at 1-800-QUIT-NOW. For those who want help quitting via their phones, Colorado Quit Mobile is a text message-based program that gives you instant support and coaching when you feel the urge to use tobacco. News from Oral Health Colorado

Oral Health Colorado is partnering with Delta Dental of Colorado Foundation on its public will building campaign, Brush With Me, that is designed to increase awareness about oral health issues, and deliver key behavior change messages to Coloradans. OHCO’s team at Miles Consulting attended the SMART Act briefings from CDPHE, CDHS, and HCPF. All three departments presented to member of “Joint Health Committee,” which included members of the Senate Health and Human Services Committee, House Public Health Care and Human Services Committee and House Health Insurance and Environment Committee. C4HCO – Progress on Enrollments

“As we enter our second month of operations, I am pleased to report several new developments. On Monday night, we added technology to our shopping website that includes the ability for customers to apply online for premium tax credits on their own after they’ve received a Medicaid denial from the State. This technology release made an immediate positive impact on the shopping experience and pace of enrollments. We also began the process of sending real-time, industry-standard and secure customer enrollment data to carriers. The shopping experience and our support operations continue to improve each day. You can also look for improvements to the Medicaid application process by mid-December as we collaborate with our State partners toward the shared mission of getting Coloradans signed up for coverage.

We’ve also added a new feature on our website that highlights customer stories and the ways in which we have created a marketplace just for Colorado, where thousands have already signed up. We encourage you to let your communities know about these customers, including their tips for new customers. Successful enrollment stories are also surfacing in the media and with our partners, including a national NPR story that aired earlier in the week. Thousands of Coloradans have signed up for 2014 coverage and we are spreading that message in our advertising campaign, which was produced by Coloradans for Coloradans and is currently airing on television, radio, online, and through text messages and billboards.

PEAK Updates

Make Medical Payments Online: The ability to make medical payments online will be available beginning November 18, 2013. On Friday, November 25th, 2013 at 10a.m., a live webinar is offered in support of this new feature. Click here to sign up for the webinar. An Instructional Guide and recording of the webinar will also be posted on the PEAK website.

Enhancements to Assets and Expenses Questions: In response to feedback, beginning November 8, 2013, only those applicants who indicate that the following criteria applies to someone in the household, will need to answer questions related to Liquid Assets, Other Assets, and Other Expenses: Blindness or Disability; Help with Self-Care; Medical/Nursing Facility Information; Future Medical/Nursing Facility Information; Over 65 years of age.

Order or Print Medical Cards Online: Clients are now able to print or request medical cards online. A five minute “How To” recording is available here, and the Instructional Guide will be posted to the PEAK website.
