Invitation to Participate: Development of Kaiser Permanente Colorado’s Community Health Needs Assessment Implementation Strategy Plan

"Several weeks ago, Kaiser Permanente Colorado conducted the first phase of an online process called Concept Mapping to inform the Implementation Strategy portion of our Community Health Needs Assessment. During that phase, over 200 public health professionals from across Colorado brainstormed strategies to address six different health needs that had been identified in Kaiser Permanente Colorado's Community Health Needs Assessment report. These health needs are: healthy eating and active living, economic stability and vitality, mental health, access to primary and specialty care, climate change, and substance use. We are now asking for your continued input to inform Kaiser Permanente Colorado as we create our Implementation Strategies that will guide our work over the next three years.

Our goal is to reach public health professionals and others with interest in population health across Colorado who can help us prioritize the strategies identified in the first phase. You do not need to be an expert in all of the identified health needs.

YOUR INVOLVEMENT
Attached is a description of the process we will use to collect information for this project. We invite you to read over how the project works, and how your participation is critical to developing a meaningful and impactful Implementation Strategy Plan. We realize that your work schedule is demanding, but hope that you will agree to make this small time investment and provide your input.

We would be very grateful if you would take just 30-45 minutes to rate the 100 unique strategies that were brainstormed during the first phase of this process. You will be asked to rate each strategy on its importance to improving the health of Coloradans and the feasibility of implementing this strategy in the next 3 years. Please complete this exercise by 5pm MT, August 4, 2016.

Colorado Bridge Program will be accepting applications for 2016 open enrollment

November 1, 2015 – January 31, 2016 Colorado Bridge will be accepting new applicants: Kaiser Permanente’s Colorado Bridge Program is a charitable health coverage program. This program is for low income individuals and families who do not have access to any other coverage and meet program eligibility requirements. Colorado Bridge provides a Kaiser Permanente premium subsidy and medical financial assistance to support cost sharing, and enrollment in an off exchange Kaiser Permanente Individual and Family Plan. Program design and premium amounts are subject to change.

Colorado Bridge Program Eligibility:
  • Must live in a Kaiser Permanente Colorado service area. The Colorado Bridge Program will be available in all Colorado service areas (Denver/Boulder, Southern Colorado, Northern Colorado, and Mountain Colorado)
  • Must have income at or below 300% of the Federal Poverty Level (FPL)
  • Must be under the age of 35 years at time of plan effective date
  • Must not have access to any other public or private health coverage including, but not limited to: Medicaid/CHP+, Coverage through Connect for Health Colorado, Medicare, job based health coverage

Kaiser Permanente Colorado - Request for Proposals to decrease food insecurity

Kaiser Permanente is excited to announce the release of a request for proposals (RFP) to decrease food insecurity. Kaiser Permanente is investing up to $1,000,000 over two (2) years with awards up to $100,000 per organization. Food insecurity refers to the degree to which food intake is reduced or normal eating patterns disrupted because of lack of money and other resources.  Intent to Apply Due: September 30, 2015. Applications Due: October 31, 2015. Grant Period: March 1, 2016-March 1, 2018.

The outcomes of this funding opportunity are:
  • Increase enrollment in Supplemental Nutrition Assistance Program (SNAP) 
  • Increase participation in the Summer Food Service Program
Organizations may submit proposals that focus on one or both of these outcomes by implementing best practices.

Eligible organizations must be a 501(c)(3) with an annual operating budget of $1.5 million or less or a Colorado health care Safety Net organization with an operating budget of any size. To be considered for funding, organizations must also meet specific eligibility criteria outlined in the RFP.

Organizations interested in applying for funding should review our frequently asked questions and read the RFP in its entirety. Kaiser Permanente - Colorado Bridge Program Closure

Effective June 15, 2015 Colorado Bridge is closed to new applicants: Kaiser Permanente’s Colorado Bridge Program is a charitable health coverage program. This program is for low income individuals and families who do not have access to any other coverage and meet program eligibility requirements. Colorado Bridge provides a KP premium subsidy and medical financial assistance to support cost sharing, and enrollment in an off exchange Kaiser Permanente Individual and Family Plan.

Colorado Bridge Program Closure: Kaiser Permanente Colorado will be re-evaluating the Colorado Bridge program design and is closed indefinitely to new applicants in all Kaiser Permanente Colorado service areas.  Current Colorado Bridge members that continue to meet program eligibility requirements will receive 2016 recertification information.

Applications for the Colorado Bridge Program have been removed from the FindYourPlan.org website and only applications received by June 15, 2015 will be processed.  Please recycle any existing Colorado Bridge Program flyers and applications your organization may have.  You will receive a Kaiser Permanente update when the Charitable Health Coverage program is accepting new applications.

Questions & Answers:
  1. Why is the Colorado Bridge Program closed to new applicants? The program is closed while Kaiser Permanente Colorado re-evaluates the program design with the goal of serving more members. 
  2. How long will the Colorado Bridge Program be closed? The Colorado Bridge Program is closed indefinitely. 
  3. Is the Colorado Bridge Program closed until open enrollment? No, the program is closed year round, including 2015 open enrollment. We will continue to evaluate the program for consideration in the future. 
  4. Can applicants with a Special Enrollment Period (SEP) still apply? No, there is no SEP available while the program is closed and Kaiser Permanente re-evaluates the program design. 
  5. Will applicants who apply for Colorado Bridge be informed the program is closed? Yes, applicants that apply while the program is closed will receive a denial letter informing them that the program is closed to new applicants. 
  6. Will this affect existing members? No, this will not have an impact on members currently enrolled in the Colorado Bridge Program who continue to meet program eligibility criteria. 
  7. Will existing members be eligible for a subsidy recertification? Yes, existing members will receive recertification information in the mail and provided an opportunity for a KP premium subsidy recertification if they continue to meet eligibility criteria. 
  8. Can dependents be added to an existing Colorado Bridge plan? Yes, dependents may be added to an existing plan as long as the dependent meet program eligibility requirements. 
  9. What if an existing member no longer meet eligibility criteria? When members no longer meet eligibility requirements, they will receive a letter explaining that they are no longer eligible for the program and will lose the Kaiser Permanente subsidy and medical financial assistance. However, enrollment in (insert name of plan) will continue but the member will be responsible for the full cost of their health plan premium and any out-of pocket expenses they incur. 
  10. If existing members lose coverage will they be reinstated or given the opportunity to re-apply? No, members that have lost coverage for failure to pay premiums or otherwise will not be reinstated nor given an opportunity to re-apply.

Safety Net Clinical Support Staff Loan Repayment Awards

Congratulations to the 25 recipients of the Kaiser Permanente Educational Loan Repayment for Safety Net Clinical Support Staff Program, including 3 staff members from ClinicNET affiliates Marillac Clinic, Clinica Colorado and Rocky Mountain Youth Clinics! Funded by Kaiser Permanente Community Health Fund, the program is designed to expand and modernize the safety net workforce through loan repayment and skill development webinars for clinical support staff. In October 2013, the Colorado Community Health Network, in partnership with Colorado Rural Health Center and ClinicNET, was funded to administer this loan repayment program in order to improve retention of health professionals who care for underserved Coloradans and provide affordable training development webinars for entry and mid-level staff.

During the initial loan repayment application cycle in January 2014, 26 clinical support staff with were awarded loan repayment funds to pay off education debt. Applicants are eligible to receive up to $10,000 in loan repayment, dependent on their completed years of service at their place of employment. Input Requested: Kaiser Permanente Food Insecurity Screening Survey by June 5

Kaiser Permanente is currently working with Hunger Free Colorado to identify patients that screen positive for food insecurity.  This has enabled 78% of referred patients to be connected to food resources. We would be honored to learn from our community partners, who we recognize may have greater expertise in this work, and welcome the chance to understand if you have a role in your community regarding hunger screening and resources. We would like to understand if your organization is screening your community for food needs and hunger challenges (aka 'food insecurity'). Here is more data about hunger in our state and its relation to health:
  • One in seven Coloradoans and one in five families with children in Colorado struggle with having enough money to buy food during the year. (USDA Household Food Security 2013) 
  • Colorado ranks 45th in the US for residents qualifying for food assistance receiving assistance. 
  • One in three US adults with chronic illness have difficulty affording food, medicine or both. (Treat or Eat: Am Journal of Med: April 2014 Volume 127) 
  • Food insecurity is associated with higher incidence of diabetes, cardiovascular disease, iron deficiency, cognitive delays, anxiety and depression.
