Meaningful Use Attestation Site Set to Open Tomorrow

"The Colorado Registration & Attestation site for Program Year 2015 Eligible Professionals is set to open tomorrow, April 29th. The site will close on June 30th, so we recommend you access it immediately and start reviewing the requirements.
For those of you who have used the site in the past, please be advised some areas have changed:

  • There are no longer Core and Menu measures; there are now 10 objectives.
  • There are exclusions and alternate exclusions for program year 2015 depending on what stage a provider is in.
  • There is now one consolidated public health objective with three measure options.

Interactive Learning Series on Pharmacy for Clinicians

Key performance measures for patient-centered medical homes include medication-related issues. Clinical pharmacists have unique knowledge and skills that can complement the care provided by other PCMH team members.

The learning series is designed to help clinicians identify how to incorporate clinical pharmacists in their practices. Conducted through ECHO Colorado in live web-based, video-conference sessions, participants use a smart phone, tablet or laptop with a camera to virtually attend each 60-minute session. A subject matter expert will lead a presentation (15 minutes) and participants present a case for feedback, recommendations and discussion.
Topics include: Clinical Pharmacist scope of practice; Collaborative Drug Therapy Management; Financial models and benefits and Integration models.
Choose either of two 5-week sessions: Friday mornings, 7:00-8:00am, May 20th - June17th or Wednesdays, noon - 1:00pm, May 25th – June 22nd.

As part of The Colorado Health Foundation's strategy to strengthen the delivery of comprehensive, person-centered primary care, this funding opportunity will build capacity in primary care clinics for the implementation of telehealth services. Funding will support up to $100,000 for costs associated with the purchase of telehealth equipment and/or the employment of workflow or billing consultants. Private practices may also apply for low interest loans.  Applications need to focus on:
  • Teleconferencing between providers and patients
  • Telemonitoring to increase regular contact with care providers and monitor conditions
  • Other innovative telehealth programs that increase access to comprehensive, coordinated primary care services through health-related technology

ECHO Colorado Learning Community Offerings

Think HIV isn’t a problem in your community? Think again.
-Many of the risk factors that put persons at risk for HCV are the same risk factors that put someone at risk for HIV.
-Injection drug use is on the rise statewide. Intravenous drug use is spiking in rural communities across Colorado and the nation due to the challenges of acquiring prescription medications and the accessibility and cost of illicit drugs such as heroin.
-HIV prevention is important in every community. By providing testing, linking individuals to care, and providing support, you are providing them with the tools they need to protect themselves and minimizing the risk to others.

ECHO Colorado is offering a free learning community to provide training and assistance that will prepare you with patient talking points, assessment tools, treatment plans and the latest information on pre-exposure prophylaxis, or PrEP. Utilizing web-based, video-conferencing technology, you can use your smart phone, tablet or laptop with a camera to connect to each live session. Sessions will consist of a presentation from a subject matter expert followed by participant presentation of cases with recommendations and discussion among the panel and participants. The Family Leadership Registry is a tool to help you connect with a Family Leader who is trained and equipped to offer the parent/family perspective.

The Family Leadership Registry is a data base of over 500 graducates of Colorado's Family Leadership Training Institute (FLTI).  These family leaders have had over 90 hours of classroom instruction on leadership and the civic process, representing diverse perspectives from across the state.  Many are eager to share their expertise with organizations seeking the parent/family perspective on issues such as weelness, busllying, early childhood, health inequity, disability and suicide prevention.

Primary Care Medicaid Rates 16/17

You may recall the Governor's proposed FY 2016-17 budget did not include the continuation of the enhanced payments for Medicaid primary care services. However, thanks to the grassroots advocacy, media efforts, and direct lobbying of the Colorado Primary Care Alliance (a coalition of 20+ organizations), within the last couple of weeks the Legislature has authorized the transfer of $20 million in funds for the continuation of rate enhancements (87.3% of Medicare) for the following selected codes:
  • Office Visit  (99201-99205, 99211-99215)
  • Preventive Medicine visits  (99381-99397)
  • Newborn (99460-99465, 99477-99486) 
  • Immunization Administration (90460-90461, 90471-90474)
  • Counseling and Health Risk Assessments (99401-99414, 99417-99429)
  • Critical Care Visit - Neonatal and Pediatric (99466-99476)
These funds are one-time and not built into the Medicaid rate base. Be sure to read HCPF's May 2016 Provider Bulletin for an official announcement!  The Alliance will be meeting to debrief on this year’s efforts and develop a go-forward strategy for FY 2017-18. April 18 Application Deadline for the Colorado Health Symposium Scholarship

The Colorado Health Foundation is launching a scholarship program to offer increased opportunities for individuals from Colorado to attend the 2016 Colorado Health Symposium.

The Symposium is 
July 27-29 at the Keystone Resort & Conference Center. This year’s theme, “Health is Everyone’s Business” builds on the Foundation’s recent statewide listening tour. We know that when people are healthier, their communities prosper. From the doctor’s office to the streets we live on, community is where health happens. 

Who Should Apply:
Colorado p
rofessionals from rural-based, minority-led and/or community-based and advocacy organizations, and industries – such as environmental, housing and education – are especially encouraged to apply. View the application and complete the online form by Monday, April 18.

All Scholarships Include:

  • Fully-waived registration fee
  • Travel stipend for anyone located more than two hours away from Keystone
  • Lodging in Keystone (for two or three nights, depending on travel distances). Lodging for scholarship recipients will be close to the conference center with free shuttle transportation if needed.

Funding Opportunity for Dental Program Innovation

The Dental Trade Alliance Foundation is now accepting applications for 2016 grant programs! If your organization has an innovative project designed to improve oral health care for the growing number of needy Americans and would like to be considered for a grant, please review the guidelines. Each applicant must demonstrate that their innovative project will not only improve access to oral care in their immediate area, but also have the potential to grow and be implemented in other areas of the country. Calling Colorado Lay Navigators

Participants are needed for a focus group.  Be a part of a nationwide research study being conducted with patient navigators. The Patient Navigation Training Collaborative (PNTC) is collaborating with researchers in other states to conduct focus groups around the country on patient navigator skills and knowledge needed for the patient navigator role. This will help inform patient navigator training at a national level. Please help us represent patient navigation in Colorado by participating in one of two focus groups offered. We are looking for lay patient navigators to participate in a study that will help us make recommendations for national patient navigation training.
 
Study Purpose: To conduct focus groups to better understand what patient navigators need to know to do their jobs.

Actively Recruiting for EvidenceNOW Southwest

The eligibility requirements for practices interested in ENSW have been expanded to include practices with up to 15 providers. In order to be eligible, practices still need to have the ability to make site-level decisions regarding guidelines, protocols, and care processes, and should not be receiving centralized quality improvement support as part of a health system. This change should open up ENSW to more practices that would have been previously excluded.

Recruitment is open and the ENSW team is accepting applications from eligible practices. Practices ready to begin ENSW activities can begin work anytime between now and September, as we are moving towards a “rolling” admission process for ENSW. Practices wanting more information can join regularly scheduled informational webinars, every Thursday starting April 14, alternating between 7:30 AM and 12:30 PM.

ACC Chronic Pain Disease Management Program

The second year of ACC Chronic Pain Disease Management Program will start in April and May with two options:

o Chronic Pain Telehealth Program: Connect PCMPs with a multi-disciplinary team of chronic pain specialists, including behavioral health professionals and pharmacists, to review client cases and learn evidence-based interventions for treating clients with complex conditions.

o Buprenorphine Telehealth Program: Providers licensed to prescribe Buprenorphine/Suboxone will connect with specialists to gain greater insights and experience in treating clients with opioid addiction.

Transforming Clinical Practices Initiative (TCPi)

The Transforming Clinical Practices Initiative (TCPi) is a four-year funding opportunity from CMS that will provide free technical assistance and coaching to health care providers, practices, and health care systems across the state, to support and prepare for the forthcoming changes in the health care payment and delivery system. This support will be tailored to the needs of every participant, but the goals are common for all – improved efficiency of care, improved health outcomes, and improved patient care experiences.  Two informational webinars are still available, click here to register.

An important consideration for Denver ClinicNET affiliates is that TCPi is willing to form specialized groups of practices that are participating in the Mile High Health Alliance's future Specialty Care Referral Network (click here for a link to their Implementation Plan), to support them in areas such as developing care coordination services and  better capacity for effective referrals. However, the practices would have to get into the original, single TCPi cohort before the deadline. Registration to become a TCPi cohort is currently open through June 2016.  Note that both primary care practices and specialty care practices can apply for support through this program.

2016 National Health Service Corps Site Application Now Open!

The 2016 NHSC New Site application cycle is now open for eligible health care sites. The application cycle will close on June 7, 2016, at 11:59 p.m. ET.  Interested sites are encouraged to read the 2016 NHSC Site Reference Guide for all program requirements. Eligible sites include health care facilities that provide outpatient, ambulatory and primary health care services (medical, dental, and behavioral) to populations residing in high-need urban and rural areas.

Although we will certainly miss her dearly, please join ClinicNET in wishing our Program and Communications Coordinator, Johanna "JoJo" Gelderman best wishes as she moves on to her next career opportunity as a Quality Improvement Coordinator with our colleagues at the Colorado Community Health Network! (Her last day at ClinicNET was April 6.)

We are happy to announce that Anne Taylor will be joining ClinicNET as our Program and Communications Coordinator. Anne is no stranger to ClinicNET or community safety net clinics (CSNCs). She comes to us after 5+ years at Rocky Mountain Youth Clinics. In 2010, she was part of the ClinicNET team and was on the original committee that established ClinicNET when she was the Executive Director of Doctors Care. She feels fortunate to have the time and ability to return to ClinicNET where she can be of service to the valuable work of CSNCs. Anne's proudest role is being mother of Will (22) and Kyle (20) and her free time is spent drawing, painting and interior design. Caring for Medically Complex Children ECHO Learning Community – REGISTER TODAY

ECHO Colorado still has room in the Caring for Medically Complex Children ECHO Learning Community that starts April 27th.  Providers will get information to better access and manage the care for children with medical complexity, create a shared care plan, engage with other types of providers involved in the care of a child with medical complexity and identify and share resources with peers. ECHO Colorado is also hosting a learning community on HIV starting May 19th.  HIV prevention is important in every community. By providing testing, linking individuals to care, and providing support, you are providing them with the tools they need to protect themselves and minimizing the risk to others.

This free learning community will provide training and assistance that will prepare providers with patient talking points, assessment tools, treatment plans and the latest information on pre-exposure prophylaxis, or PrEP. Utilizing web-based, video-conferencing technology, you can use your smart phone, tablet or laptop with a camera to connect to each live session.  Sessions will consist of a presentation from a subject matter expert followed by participant presentation of cases with recommendations and discussion among the panel and participants. Provider Revalidation (Medicaid, Medicare & CHP+) deadline: September 24, 2016

By completing the revalidation/enrollment process now through the Online Provider Enrollment (OPE) tool, providers will not experience any delay in payment when the new enrollment and claims management system, Colorado interChange, launches on October 31, 2016. Starting on that date, claims and encounters submitted by providers who have not enrolled and/or revalidated will be denied.

Please do not begin the application before reviewing all of the training resources available online. An incorrect or incomplete application requires additional review, which may add weeks to processing time.  Revalidation and Enrollment instructions are available online. Be sure to review the Information by Provider Type before you begin the online training, as it will help you select the correct training. The Provider Enrollment Manual also includes valuable information to help providers complete applications correctly.

Colorado PEAK Updates

New! PEAK Monthly Training Tips: Join the PEAK Outreach Team on Wednesday, April 20, as we kick off a new series of monthly web conferences. Informed by the needs of our community partner network, each session will provide the opportunity to engage in Q & A and share best practices with other organizations. The topic for April will be Reporting a Change. The agenda will include time for updates from the PEAK Outreach Team and any news participants have to announce. Click here to register.
