"This survey is being conducted to collect input from Medicaid consumers about their preferences for receiving primary care in the Accountable Care Collaborative (ACC) program.  All surveys must be complete by Wednesday, December 4th, 2013 (online survey closes at 5:00pm). Please note that the deadline has been extended from November 29th to December 4th.

This survey is being conducted by a workgroup of the Provider and Community Relations Subcommittee, which is part of the ACC Program Improvement Advisory Committee.  The results will be used to inform discussion about potential policy recommendations to the Department of Health Care Policy and Financing.  The survey may be completed by an adult Medicaid recipient for his or herself and for each of his or her children who receive Medicaid. PHI Survey: Leveraging Mobile Phones to Engage Safety Net Patients

"The Public Health Institute has received a grant from the Commonwealth Fund to conduct a national assessment of the current status of deployment of mobile technology within the safety net in better serving the needs of their populations.  As health systems adopt more patient-centered care models under health care reforms, improved understanding among providers as to how mobile technology may be used effectively is required.

The survey will collect data from provider organizations on their use of mobile phones in engaging and better serving their clients' care needs.  Patient engagement, which involves actions that people can take to improve their health, can benefit from tools such as mobile phones and the information and other resources they provide to support a patient's motivation to change their behaviors in ways that can lead to improved health outcomes.

The survey should take no longer than 15 minutes to complete.  The participation of organizations that do not currently use mobile technology is also encouraged.  The data collected through the survey will provide evidence-based findings that promote understanding among providers as to how mobile technology may be used more effectively as well as to develop recommendations that advance health solutions in the field.

Survey: Pregnancy Related Depression Resources in Colorado

The Maternal Wellness team at the Colorado Department of Public Health and Environment is surveying individuals, organizations, programs and providers around the state to identify what screening, referral and treatment/support efforts are available for depression that occurs either during pregnancy or up to one year postpartum (also known as pregnancy-related depression).

If you are a safety net clinic that:

•      Provides services to pregnant or postpartum women; or
•      Provides women's health services; or
•      Provides early childhood services

then you are encouraged to respond to the survey and/or share it to others. The survey should take between 10-15 minutes to complete.

The survey will be open through Wednesday, January 30th.

Take this survey on the 9/12 COHBE Public Meeting

On September 12, 20122 the Colorado Health Benefits Exchange (COHBE) invited members of the public to attend a meeting to view and provide feedback about preliminary customer shopping screen designs for individuals and families.  COHBE is distributing an online survey for Coloradans to review and submit feedback about preliminary shopping screens. Please submit the survey by 5 pm on September 18.
