National Diabetes Month: New Tools to Help Providers and Patients

“November is National Diabetes Month, a time dedicated to spreading awareness about diabetes and the countless people impacted by it. The American Diabetes Association has partnered with the American Association of Diabetes Educators and the Academy of Nutrition and Dietetics to publish a joint position statement about the importance of self-management, education, and support for patients living with type 2 diabetes or prediabetes.

The National Diabetes Education Program is also promoting new tools for providers. They’ve highlighted 10 guiding principles designed to help providers deliver high-quality care to patients with or at risk for diabetes. Colorado has also been greatly impacted by type 2 diabetes and prediabetes. Nearly half a million Coloradans suffer from the disease, and more than 1 in 3 Coloradans has prediabetes. The Diabetes Prevention Program is aimed at providing those Coloradans with the education and support they need to reduce their risk of type 2 diabetes. Colorado Medicaid Implements New Benefit for Diabetes Self-Management Education (DSME)

“The Colorado Department of Public Health and Environment (CDPHE) is providing an opportunity for Federally Qualified Health Centers, Rural Health Clinics and safety net clinics to acquire accreditation of their Diabetes Self-Management Education (DSME) program through either the American Diabetes Association (ADA) and the American Association of Diabetes Educators (AADE) to ensure that DSME programs meet national standards.

The toolkit and scholarship application are available on CDPHE’s Diabetes Partner Resources. All applications should be received by Nov. 30, 2015 to be considered.” [Source: CDPHE]

Diabetes Self-Management Education Policy and Accreditation Scholarship Application

“The Colorado Department of Public Health and Environment is currently providing an opportunity primarily for Safety Net Clinics, Federally Qualified Health Centers and Rural Health Centers (though others with billing ability and high need will be considered) to acquire accreditation of their Diabetes Self-Management Education (DSME) program through either the American Diabetes Association (ADA) and the American Association of Diabetes Educators (AADE) to ensure that DSME programs meet national standards.
  • Consideration of applicants will be based on a number of factors (listed in the application) and the number of requests received.
  • CDPHE support will cover the cost of accreditation only. 
  • Please note all applications should be received by November 30, 2015 to be considered.

Increasing Access to Diabetes Self-Management Education

“Diabetes self-management education/training (DSME/T) is an evidence-based critical point of care program for people with diabetes that can delay or prevent diabetic complications. The objectives of DSME/T include:
  • Support informed decision making
  • Promote self-care behaviors
  • Initiate problem solving
  • Promote active collaboration with the health care team
  • Improve clinical outcomes, health status, and quality of life
Data published in the CDC's Morbidity and Mortality Weekly Report indicate that less than 7% of privately insured adults with newly diagnosed diabetes from 2009 to 2012 joined a self-management education and training program. Researchers found that older adults and insulin-treated patients were more likely to join such initiatives.

National Diabetes Education Program Tools to Help Promote National Diabetes Month

“November is National Diabetes Month, and the National Diabetes Education Program (NDEP) has developed a variety of promotional tools, including diabetes management resources, key messages, template news release and feature articles, YouTube videos, and more.

Throughout National Diabetes Month, NDEP and its partners will promote “Be Smart About Your Heart: Control the ABCs of Diabetes” to help people with diabetes learn they are at greater risk for heart disease, and also how they can lower that risk by managing the diabetes ABCs: A1C test, Blood Pressure, Cholesterol and Stop Smoking.

Lifestyle Change programs for individuals with diabetes and prediabetes are cost effective

“The average medical expenditures among people diagnosed with diabetes were 2.3 times higher than people without diabetes, according to the 2014 National Diabetes Statistics Report.

A recent study conducted at Wake Forest Baptist Medical Center found that participants who were assigned to an intensive lifestyle change program focused on diet and exercise experienced statistically significant average savings compared to individuals assigned to a standard diabetes support program. Financial savings stemmed from fewer hospitalizations, shorter hospital stays, and a reduction in prescription medication usage.

The Community Preventive Services Task Force released an evidence-based recommendation in favor of programs that actively encourage those at increased risk for type 2 diabetes to eat healthier and be more physically active. The Task Force also concluded from the economic evidence that combined diet and physical activity promotion programs to prevent type 2 diabetes among people at increased risk are cost-effective.

Diabetes Prevention Program lifestyle coach training offered

“Viridian Health Management is offering online training classes to become a Lifestyle Coach to deliver the Centers for Disease Control and Prevention National Diabetes Prevention Program. The training is a dynamic blend of independent learning and weekly live Web-based discussions facilitated by a Viridian Master Trainer. Once registered, participants will receive instructions on how to access the online training platform. Throughout the online training, participants will have 24/7 access to technical support.

CDPHE & CBS4 Prediabetes Spanish Language Campaign

“One in three U.S. adults has prediabetes; most are unaware they are at risk. CBS4 is continuing its prediabetes campaign to raise awareness through May 2014. The campaign is designed to educate consumers about risk factors that lead to type 2 diabetes and drive enrollment into the Diabetes Prevention Program.

The campaign is targeting Spanish speaking consumers in Denver. CBS4 has partnered with Entravision (Univision TV, KPXk and KJMN Radio) to produce Spanish language television and radio spots.  Spanish speaking consumers are directed to a custom website, which provides educational information on prediabetes risk factors, and encourages a call to 1-800 Diabetes to learn more. CBS Outdoor assets at bus shelters, outdoor benches and billboards  further extend the campaign reach and can be seen around the Denver metro in various locations. DSME Advisory Work Group: March 7th

“The American Diabetes Association and the Colorado Department of Public Health and Environment would like to invite you to join a DSME Advisory Work Group interest meeting. The group will work to identify gaps, barriers and opportunities to improve diabetes self-management and diabetes prevention.

Our first meeting will be a Happy Hour following the American Diabetes Association’s Advance Diabetes Practice for Primary Care Conference On Friday, March 7th from 3:00 p.m.-4:45 p.m. at the Courtyard Marriott Denver-Cherry Creek.  The Happy Hour/DSME Advisory Work Group is FREE and does not include admission to ADA Conference. March 10 and 11 Diabetes Prevention Program Lifestyle Coach Training in Denver

“Is your organization planning on delivering the National Diabetes Prevention Program lifestyle change program – the nation’s foremost evidence-based program for the prevention of type 2 diabetes?

The Colorado Department of Public Health and Environment is partnering with the Diabetes Training and Technical Assistance Center (DTTAC) at Emory University to bring National Diabetes Prevention Program Lifestyle Coach Training to Colorado on March 10 and 11, 2014.

Who is a candidate for diabetes prevention?

ADA Offers Free mHealth Program: Living With Type 2 Diabetes

"To guide and support individuals with type 2 diabetes, the American Diabetes Association is offering a free 12-month program called Living With Type 2 Diabetes.  The program provides information and offers a free guidance to help people learn how to manage diabetes at regular intervals throughout the year-long.

The program is available in English and Spanish and participants can choose to receive information online or through the mail.  Participants can also choose to receive additional recipes, tips and reminders on their phone.  People can enroll into this free program by visiting the website, calling 1-800-DIABETES, or texting Type2 to 69866 to learn more about the program in English or Tipo2 to 69866 to learn more about the program in Spanish.
