Register Today: Webinar for TCHF Strengthening Primary Care Funding Opportunity

"With the passage of the Affordable Care Act (ACA), Medicaid expansion and the growing momentum towards outcomes-based payment models, Colorado primary care providers are adapting to a changing environment. Debt can be a strategic and financially viable way to support primary care clinics’ one-time and ongoing resource needs connected to adaptation. This 90-minute webinar, “An Introduction to Debt & Financing for Colorado Primary Care Clinics” facilitated by Nonprofit Finance Fund is intended for primary care clinic staff and board members interested in applying for the Strengthening Primary Care funding opportunity. It is structured to provide an understanding of the key opportunities and risks associated with the use of debt to support capitalization needs. The training includes content that will provide participants with:
  • A greater understanding of the types of loans that primary care clinics commonly utilize;
  • Best-practices in accessing debt and its strategic uses; and 
  • Approaches to communicating resource needs and preparing for requests for debt (i.e., roles of people, processes and financial reports).
This webinar will be hosted by Alice Antonelli, advisory services director at Nonprofit Finance Fund, on Wednesday, Sept. 28 from 12:30-2 p.m. Colorado-based primary care clinics interested in the Foundation’s Strengthening Primary Care funding opportunity are encouraged to join."

TCHF Funding Opportunity: Strengthening Primary Care

This initiative is intended to support primary care providers by offering low-interest loans in addition to grants so that organizations can access the flexible capital they need to increase access to and improve the delivery of comprehensive, person-centered care. Grants of up to $500,000 and/or low-interest loans in the range of $500,000 to $5 million are available to clinics for physical and information technology infrastructure as well as working capital related to expansion or improvements to care delivery.

Colorado organizations providing primary care services may apply for funds to support the following projects:
  • Facility acquisition, construction, renovation or expansion
  • Medical and office equipment purchases
  • Hardware, software, installation and technical assistance costs associated with health information technology systems
  • Working capital for expenses associated with new or expanded services
Nonprofit entities are encouraged to consider applying for loans or a mix of grant and loan funding if appropriate. Applicants may indicate their interest in applying for a low-interest loan within the grant application. Register for an upcoming webinar, “An Introduction to Debt & Financing for Colorado Primary Care Clinics” by the Nonprofit Finance Fund for more information on key opportunities and risks associated with the use of debt to support capitalization needs.

The Foundation is partnering with Vital Healthcare Capital (V-Cap) to provide low-interest loans. V-Cap will accept loan applications on a rolling basis.

TCHF Funding Opportunities

Primary Care Leadership
As a part of the Foundation's primary care strategy, this initiative will fund programs supporting the development of primary care clinician champions who advocate for comprehensive, person-centered care at the practice, community, state or federal level. The goal of this new funding opportunity is to enhance the primary care workforce to drive change, strengthen the delivery of services and improve the health outcomes of those served.

Nonprofit organizations inside and outside of Colorado will be considered. Awarded programs must be designed with a focus on Colorado's health care system and participants must be primary care clinicians (including physicians, nurse practitioners, physician assistants, registered nurses, licensed behavioral health provider, dental hygienists and dentists) practicing in the state. Priority will be given to programs that target clinicians serving rural and low-income communities and culturally represent the patients they serve.

Activating Places and Spaces Together
As part of the Colorado Health Foundation's focus on ensuring that communities have activated, quality indoor and outdoor infrastructure that encourages kids to be physically active, this funding opportunity supports locally-defined, place-specific efforts to get people outdoors and actively engaged in their neighborhoods - together. The goal of the funding opportunity is to help activate existing infrastructure in public places that contributes to a community's overall health through residential usage and positive experiences. Foundation grant funds will support costs associated with project/program planning and/or implementation for up to one year.

TCHF: Telehealth Funding Opportunity

As part of the Foundation's strategy to strengthen the delivery of comprehensive, person-centered primary care, this initiative will build capacity in primary care clinics for the implementation of telehealth services. Funding will support up to $100,000 for costs associated with the purchase of telehealth equipment and/or the employment of workflow or billing consultants. Private practices may also apply for low interest loans. 
The Foundation seeks applications that focus on: 
  • Teleconferencing between providers and patients
  • Telemonitoring to increase regular contact with care providers and monitor condition.
  • Other innovative telehealth programs that increase access to comprehensive, coordinated primary care services through health-related technology
This funding opportunity is designed to increase access to comprehensive, coordinated care through telehealth services. To be considered for funding, applicants must meet specific criteria.

The Special Projects Program is designed to address those needs and to help TCHF learn over time how they can better support and invest in projects that are important to achieving the shared vision of a healthiest Colorado.

Webinar: Harnessing the Power of Advocacy in Your Nonprofit

"There are many opportunities for 501(c)(3) nonprofit organizations to advance their missions by engaging in advocacy to influence public policy. In fact, advocacy can be an effective catalyst for policy changes that leverage nonprofits' program work and carve a path towards mission advancement and sustainability. Nevertheless, even 501(c)(3) organizations with substantial advocacy experience and expertise can have questions about the legal parameters governing how their organizations can engage in specific advocacy activities like voter outreach, lobbying or policymaker engagement.

To help clarify legal considerations for 501(c)(3)s that engage in advocacy, the Colorado Health Foundation is sponsoring an upcoming webinar on August 11th, “Harnessing the Power of Advocacy in Your Nonprofit.” This webinar will guide participants through the process of integrating advocacy activities into their organization's activities whether they are experienced advocates or new to advocacy. Cara Lawrence, partner at the law firm of Tierney Lawrence, LLC, will lead the discussion and share information to help participants understand both the opportunities advocacy can present to advancing your organization's mission and the legal compliance issues 501(c)(3) organizations must consider when engaging in advocacy.

"Adapting to a Changing Landscape: Colorado’s Safety Net Health Centers"

"Driven by the desire to better support Colorado health centers in the midst of rapid adaptation, The Colorado Health Foundation partnered with Nonprofit Finance Fund (NFF) to take a critical look at how these organizations are faring as their business models evolve in the face of systemic shifts."..."NFF conducted an intensive cohort analysis to study the drivers of financial sustainability for 21 health centers, examining audited financials, payor/patient data and conducting in-depth interviews with Colorado’s health center leaders." 5 ClinicNET affiliates were included in the cohort of 21. Click here to read the findings and recommendations [Source: NFF]
