Convening Meeting of the Colorado Primary Care Collaborative, January 9th

"The Colorado Primary Care Collaborative (CPCC) would like to invite you to participate as a supporter of our exciting initiative. The CPCC is a broad coalition of heath care providers, business leaders, patient advocacy and special interest groups, insurance brokers and health plans. Your participation in the collaborative will ensure your voice in our broad coalition. To achieve our mission, we work in partnership with our members and fellow medical home advocates to promote the vision of the CPCC by:
  • Disseminating results and outcomes from medical home initiatives and clearing communicating their impact on patient experience, quality of care, population health and health care costs.
  • Advocating for public policy that advances and builds support for primary care and the medical home, including payment reform, patient engagement, and employer benefit initiatives.
  • Convening health care experts, thought leaders, and consumers to promote learning, awareness, and innovation of the medical home model.
