Survey for Medicaid Clients

"This survey is being conducted to collect input from Medicaid consumers about their preferences for receiving primary care in the Accountable Care Collaborative (ACC) program.  All surveys must be complete by Wednesday, December 4th, 2013 (online survey closes at 5:00pm). Please note that the deadline has been extended from November 29th to December 4th.

This survey is being conducted by a workgroup of the Provider and Community Relations Subcommittee, which is part of the ACC Program Improvement Advisory Committee.  The results will be used to inform discussion about potential policy recommendations to the Department of Health Care Policy and Financing.  The survey may be completed by an adult Medicaid recipient for his or herself and for each of his or her children who receive Medicaid. Only one survey should be completed per individual Medicaid recipient."  [Source:  HCPF]
