Interactive Learning Series on Pharmacy for Clinicians

Key performance measures for patient-centered medical homes include medication-related issues. Clinical pharmacists have unique knowledge and skills that can complement the care provided by other PCMH team members.

The learning series is designed to help clinicians identify how to incorporate clinical pharmacists in their practices. Conducted through ECHO Colorado in live web-based, video-conference sessions, participants use a smart phone, tablet or laptop with a camera to virtually attend each 60-minute session. A subject matter expert will lead a presentation (15 minutes) and participants present a case for feedback, recommendations and discussion.
Topics include: Clinical Pharmacist scope of practice; Collaborative Drug Therapy Management; Financial models and benefits and Integration models.
Choose either of two 5-week sessions: Friday mornings, 7:00-8:00am, May 20th - June17th or Wednesdays, noon - 1:00pm, May 25th – June 22nd.

ECHO Colorado Learning Community Offerings

Think HIV isn’t a problem in your community? Think again.
-Many of the risk factors that put persons at risk for HCV are the same risk factors that put someone at risk for HIV.
-Injection drug use is on the rise statewide. Intravenous drug use is spiking in rural communities across Colorado and the nation due to the challenges of acquiring prescription medications and the accessibility and cost of illicit drugs such as heroin.
-HIV prevention is important in every community. By providing testing, linking individuals to care, and providing support, you are providing them with the tools they need to protect themselves and minimizing the risk to others.

ECHO Colorado is offering a free learning community to provide training and assistance that will prepare you with patient talking points, assessment tools, treatment plans and the latest information on pre-exposure prophylaxis, or PrEP. Utilizing web-based, video-conferencing technology, you can use your smart phone, tablet or laptop with a camera to connect to each live session. Sessions will consist of a presentation from a subject matter expert followed by participant presentation of cases with recommendations and discussion among the panel and participants. ClinicNET needs information from your Patient Navigators or Community Health Workers about barriers to care!

The Colorado School of Public Health and The Colorado Patient Navigator Training Collaborative are launching Learning Communities through Project ECHO to support connection and partnership within the community, focusing upon CHW and PN related interests and topics.

In order to help inform this project, ClinicNET is looking for case studies (it will be de-identified) of patients who have been difficult to serve. This can be for any reason (needs are great, unable to find services, patient not following through, unable to pay for care, etc). We would be grateful to have the opportunity to speak with anyone in this position of patient navigation and/or care manager (PN/CM), as well as providers or other staff who are engaged with patients who you are struggling with overcoming the barriers presented by the patient to engage in health care treatment. Below is a list of questions to complete.  Please email your answers to cathryn.benedict@clinicnet.org or call her at 720-280-0997. In return, Cathryn will provide your clinic with resources developed through this project. Thank you in advance!

• What does the person already have access to and connections within his/her community? What is missing?
• Do close relationships act as barriers or assets?

“ICD-10 Presentation April 23rd:  The Colorado Children’s Healthcare Access Program (CCHAP) will be hosting a presentation on ICD-10 implementation for providers, managers, and staff on April 23rd noon-1pm. This was one of the most requested topics from our annual practice survey. Our presenter is a national expert on ICD-10 and we are excited to be able to bring his expertise to medical home practices in Colorado. You may attend in person or via webinar remotely. ECHO for Children and Youth with Epilepsy: Project ECHO® (Extension for Community Healthcare Outcomes) is an innovative healthcare program developed by the University of New Mexico to treat chronic and complex diseases in rural and underserved areas by supporting and expanding the primary care provider scope of practice.

ECHO Colorado will be launching spring of 2015 to support providers across Colorado and surrounding states. The University of Colorado and Children’s Hospital Colorado have partnered with the American Association of Pediatrics to offer ECHO for Children and Youth with Epilepsy (CYE) as the first ECHO Colorado topic. CYE ECHO, beginning March 3, 2015, is currently recruiting community healthcare providers who are interested in expanding their scope of practice specific to pediatric epilepsy.

During ECHO clinics, healthcare clinicians present de-identified cases using a case-based format by video conferencing equipment. Project ECHO specialists and other participants discuss patient cases and recommendations are made regarding diagnostic and treatment plans. CME credit is provided at no cost for participating (1hour = 1CME credit) conducted via teleconference.
