Overdose Awareness Day

Join us as we gather to pay tribute to those we have lost and raise awareness on what we can do to stop accidental overdoses here in Colorado on August 31, International Overdose Awareness Day.

We will meet at the Harm Reduction Action Center (231 E. Colfax Ave.) at 3:30pm and proceed to Colfax & Broadway where we will raise awareness with passerby's and hold signs to raise awareness to the drivers in that intersection. We have pushed our event to later in the day this year (4:00 - 5:30pm) in an effort to allow more to attend and in hopes to create a bigger impact by raising awareness during rush hour. We need to stop the silence in our community.

Patient Navigator Supervisor: Leading and Administering a Patient Navigation Program
Learn more about defining the PN role, how to craft job descriptions, and how to identify the training needs of your patient navigator staff.  The course also provides a basic understanding of the patient navigator intervention model, reviews challenges facing PN program managers and provides strategies for an effective PN program.  We will review identifying evidenced-based clinical guidelines utilizing registries for identifying patients, developing standard work flows, PN training, key supervision needs and evaluation of PN services.

Calling Colorado Lay Navigators

Participants are needed for a focus group.  Be a part of a nationwide research study being conducted with patient navigators. The Patient Navigation Training Collaborative (PNTC) is collaborating with researchers in other states to conduct focus groups around the country on patient navigator skills and knowledge needed for the patient navigator role. This will help inform patient navigator training at a national level. Please help us represent patient navigation in Colorado by participating in one of two focus groups offered. We are looking for lay patient navigators to participate in a study that will help us make recommendations for national patient navigation training.
 

Study Purpose: To conduct focus groups to better understand what patient navigators need to know to do their jobs. Upcoming Patient Navigator Trainings

“Level 1: In February, Level 1 will be offered in both Pueblo and Denver.  In March, we’ll take Level 1 north to Fort Collins. In the spring, Level 1’s will be offered in Colorado Springs, on the Western Slope and in metro Denver. Dates and exact locations are TBD – stay tuned!

Level 2: Advanced Health Behavior Change will be offered in in February and April (the January course is already full, but a waitlist is available). Advanced Care Coordination will also be offered in February.

Online: For those looking for an online course, the next installment of Emotional and Social Aspects of Disease (a Level 2 course) kicks off on May 8. Volunteers needed to pilot-test scenarios with mock patients: As an experienced patient navigator, you are invited to participate in the development of a skills exam that will be used in the voluntary credentialing process for patient navigators. This will be held January 23, 2016 at the Center for Advancing Professional Excellence (CAPE) on the Anschutz Medical Campus, 13070 E 19th Ave, Aurora CO, 80045. PN volunteers will be asked to meet with up to 15 patient actors during a 3.5-hour session; available time slots are 8:30am, 11:50am, and 2:30pm.  Click here for more information.

The Patient Navigator Training Collaborative offers a full curriculum designed to prepare new patient navigators or improve patient navigation knowledge and skills. The curricula have been developed by content experts including patient navigators, healthcare providers, educators and trainers. Expert trainers lead interactive and engaging courses. Courses vary in length and some are available online. All courses offer a certificate of completion.

The Community Health Worker and Patient Navigator Work Group is a statewide coalition comprised of local and state agencies, advocacy groups, philanthropic organizations and public & private health plan representatives.  This group coalesced in an effort to align all of the various patient navigator/community health worker/promotoras groups in our state and identify and develop a sustainable model for this important work.

ECHO for Patient Navigator Health Promotion Learning Series

“Connect with other patient navigators from your home or work computer: ECHO for Patient Navigator Health Promotion Learning Series. Wednesday, October 21, 2015 –Wednesday April 20, 2016, every 3rd Wednesday of the month, 8:00 am—9:00 am (MST). REGISTER HERE!

Health promotion is the process of supporting people to increase control over, and to improve, their health. It moves beyond a focus on individual behavior towards a wide range of social and environmental interventions. This series will provide information and case study support from national subject matter experts in health promotion.

Sessions will include: What is Health Promotion, Assessing Patient Readiness, Tracking Multiple Health Conditions, Resources for Health Promotion, Making Information Relevant, Using Self-Monitoring Tools, Dealing with Patient Perceptions.

ECHO Learning Community: The ECHO participants include the subject matter expert team (hub) and community healthcare workers (spokes). During ECHO sessions, a didactic is given by the subject matter expert team. Community healthcare workers then present cases using video conferencing equipment. Participants discuss the cases and recommendations are given. “This summer the Patient Navigator Training Collaborative launched Project ECHO with Patient Navigation. Join them on September 16, 8-9am for the final Patient Resources installment to see what it’s all about! You can participate in the live video via ZOOM, by phone (Dial: 1-415-726-9988 or 1-646-568-7788; Meeting ID: 112 358 541), or in-person (Building 406 2nd floor, Center for Public Health Practice, University of Colorado Anschutz Medical Campus, 12477 E. 19th Avenue, Aurora, CO 80045). A new series will relaunch in October with the topic focus of Health Promotion.

The ECHO for Patient Navigators focused on increasing patient navigator’s knowledge and skills in topics of interest. During ECHO sessions, topic experts present brief trainings on current best practice strategies and patient navigators present cases on the application of those strategies in their practice using bi-directional video. ECHO Colorado topic experts and other participants discuss cases and recommendations are made regarding next steps. Patient navigators may bring the same cases back for ongoing decision making support.
