The Transforming Clinical Practices Initiative (TCPi) is a four-year funding opportunity from CMS that will provide free technical assistance and coaching to health care providers, practices, and health care systems across the state, to support and prepare for the forthcoming changes in the health care payment and delivery system. This support will be tailored to the needs of every participant, but the goals are common for all – improved efficiency of care, improved health outcomes, and improved patient care experiences. An important consideration for Denver ClinicNET affiliates is that TCPi is willing to form specialized groups of practices that are participating in the Mile High Health Alliance's future Specialty Care Referral Network (click here for a link to their Implementation Plan), to support them in areas such as developing care coordination services and better capacity for effective referrals. However, the practices would have to get into the original, single TCPi cohort before the deadline. Registration to become a TCPi cohort is currently open through June 2016. Note that both primary care practices and specialty care practices can apply for support through this program.
