2016-17 Primary Care Fund Remaining Deadlines

For those clinics/agencies who applied for the Primary Care Fund, please note the following deadlines:

• Intent to apply notification were due from the Applicant Agencies to the Department by Monday, April 18, 2016.
• Written questions were due from the Applicant Agencies to the Department by Monday, April 25, 2016.
• Stakeholder Conference will be held on Tuesday, May 3, 2016.
• Application responses are due by close of business on Thursday, May 26, 2016.
