Safety Net Clinic Week, AUGUST 19-23, 2013!
To jump start preparations for Safety Net Clinic Week 2013, ClinicNET and the Colorado Rural Health Center sent out “Save the Date” postcards to Colorado's Congressional delegation, State legislators and other key Colorado community members.
Highlights from last year's Safety Net Clinic
Week included 14 clinic site-visits with elected officials, governmental staffers, and candidates for office. 5 clinic open houses, and 5 governmental proclamations! Clinics that participated last year said that site-visits and open houses were a great way to
build relationships, increase
awareness of services, highlight the
importance of clinics, and acknowledge partnerships in the community.

Healthy Smiles, Healthy Children Invites Letters of Intent for 2014 Access to Care Grants
"The Foundation of the American Academy of Pediatric Dentistry's Healthy Smiles, Healthy Children initiative will award single-year matching grants of up to $20,000 to support community-based initiatives that provide oral health care to undeserved children. Priority will be given to successful programs with the potential for replication in other communities, as well as initiatives that have worked in collaboration with other institutions and organizations.
Funds may be applied to cover the cost of clinic supplies and instruments, patient/parent education materials, take-home supplies (toothbrushes, toothpaste, etc.), education and/or outreach to recruit dentist participation in program activities, and/or other activities with clear, direct impact on pediatric oral health care.
To be eligible, applicants must be a U.S.-based nonprofit organization or local government agency that provides dental services to children in the U.S. or a U.S. territory. Projects should have pediatric dentist involvement. If a pediatric dentist is not participating, a general dentist must be involved in order to be considered for funding.

Several enhancements are coming to Colorado PEAK! These enhancements will improve user friendliness and set the stage for greater connectivity in the very near future. These enhancements will impact all PEAK account holders, both existing and new. All in-process applications must be submitted prior to 5:00PM on June 21, 2013 or they will be lost. 'Apply for Benefits' Log-In Changes: The PEAK site will begin using a client's e-mail address as the user name. Clients will be able to reset their passwords without contacting the Help Desk. Clients will choose security questions to validate the identity of anyone seeking to reset the password. Unlike before, users will not have to remember their security questions as they are now linked to the user name.
Client E-mail Address: Anyone may apply for benefits using PEAK, regardless of whether they have an e-mail address. Without an e-mail address, clients will login as a guest but will not be able to save their un-submitted application. With an e-mail address, clients will be able to save an application and use the new 'Manage My Account' features.
Note: If you would like to use a 'How to Get Email' guide with your clients, you can request one from peakoutreach@bouldercounty.org
All Peak 'Apply for Benefits' applications that are in process and have not been submitted must be submitted prior to 5:00PM on June 21, 2013 or the information will be lost. Premier Cares Award
The Premier Cares Award was established to recognize exemplary programs nationwide that have made health services more accessible to a specific medically underserved population, particularly as demonstrated in superior health outcomes. The award spotlights innovative programs that support people excluded from or underserved by the mainstream health delivery system, including farm workers, homeless children, pregnant teens, low-income mothers and infants, and individuals who don't have the strength or means to reach a clinic or hospital. The winning program must be able to show measurable benefits/outcomes covering a full two-year span and must be capable of being replicated. The award winner receives $100,000 and the other five finalists receive $24,000 each. The entry deadline is July 31, 2013. [Survey] Patient Navigation, Community Health Worker and Care Coordination in CO Communities
"The Colorado Patient Navigator and Community Health Worker Collaborative
hopes to learn more about the resources available to clients and
activities underway in the area of patient navigation, community health
work and care coordination. We hope the results of this survey will help
to better coordinate services within communities through an
understanding of what is needed to support this work and the current
status of community health work, patient navigation and care
coordination in Colorado to advance the field.
As a partner in this effort, the Colorado School of Public Health is helping conduct this survey (COMIRB #13-1524).

Section I: Persons actually delivering the service/care to clients
Section II: Persons directing or managing programs

Section VI: Persons who are not currently involved but have an interest
Please provide input about ALL areas applicable to your work or services. On average, the survey will take 15 minutes to complete. If you chose to share your contact information and allow your information to be released, you will receive the results of the survey. If you do not wish your contact information to be shared, the data will NOT be provided to outside agencies. We encourage you to forward this survey along to your networks and colleagues. The Tobacco Education, Prevention, and Cessation Grants Program
"The Tobacco Education, Prevention, and Cessation Grants Program is pleased to release a Request for Applications (RFA). The purpose of this Request for Applications (RFA) is to strengthen and grow the Tobacco Education, Prevention and Cessation Grants Program funding portfolioto achieve our strategic plan goals, reduce the health disparities of tobacco use and access, and increase health equity for priority populations across Colorado.
The Tobacco Review Committee expects to recommend grants totaling approximately $3 million for the period beginning approximately Jan. 1, 2014 through June 30, 2014. The number and amount of grant awards will vary depending on the number of awarded applications, proposed activities and recommendations made during the review process.
