Outreach and Enrollment Distance Learning Series Recordings
“The Colorado Covering Kids and Families (CKF) project, as part of the Colorado Community Health Network, along with Community Health Association of Mountains/Plains States (CHAMPS) is hosting an Outreach and Enrollment (O&E) Distance Learning Series. Recordings of these webinars are available online for safety net providers. AACDP and Safety Net Solutions dental clinic survey
“The American Association for Community Dental Programs (AACDP) and Safety Net Solutions, a program of DentaQuest Institute, would appreciate your assistance in completing a short survey. We are interested in gathering information about non-federally qualified health center dental clinics and programs located across the county to address oral disease prevention, treatment and education needs of uninsured and underserved populations. These clinics and programs are a vital part of the dental infrastructure system for vulnerable populations.

Informational webinar on Project A-L-L to improve the health of diabetes patients
“A-L-L is a systematic medication protocol that has shown to reduce the risk of heart attacks and strokes in patients with diabetes by 71% after five years of therapy. A-L-L stands for Aspirin, Lisinopril, and Lipid-Lowering Medication. It is a simple and evidence-based medication bundle that align with recent guideline recommendations and can be used as a population management tool for diabetes. Since 2006, A-L-L program has been successfully implemented in over 60 community health centers and hospital systems serving over 40,000 low-income, uninsured, medically underserved and vulnerable patients with diabetes in five regions (including Colorado).
The ultimate goal of Project A-L-L is to improve the health of diabetic patients, specifically by preventing cardiovascular events (heart attack and stroke). The Colorado Community Health Network (CCHN) recognizes that health care safety nets are essential partners in our shared goals of improving the health of individuals and communities. Therefore, CCHN is committed to provide technical support in translating this best practice into safety net settings, including a change package containing tools and resources and access to clinicians from Kaiser Permanente Colorado.
CCHN will be holding an informational webinar on the A-L-L program on Tuesday, June 3rd 11:00am-12:00pm. New Health Advocacy Organization Appoints First Executive Director
“Last year, the Colorado Health Foundation created and seed funded a new, health-focused 501(c)(4) organization in an effort to bolster health advocacy in Colorado. Now that organization has a leader. The advocacy group, which will be officially named in the coming weeks, has hired Jake Williams as its executive director. As its new head, Williams will drive its mission to increase Coloradans' access to health coverage, quality health care and healthy living through changes in public policy. NeedyMeds just released a web-based, self-paced tutorial designed to teach medical office staff and other healthcare workers how to better help patients who can't afford their medications and healthcare costs. Office staff gain practical knowledge about how to access and enroll patients in assistance programs and receive a certificate upon completion of the tutorial. Dental Benefits Collaborative Update
“Adult Dental Benefit:
- You may view the final version of the Adult Dental rule, which outlines the more comprehensive adult dental benefit that will become available as of July 1, 2014.
- The Department is pleased to report that the Joint Budget Committee authorized additional funding for complete dentures in the state’s 2014-15 budget. This new denture benefit will appear in rule separate from the adult dental benefit effective July 1, 2014 and will not be subject to the yearly $1,000 adult dental limit.
- A revised version of the Dental Listening Log has been posted to reflect the proposed denture policy.
- All comments received as part of the 45 Day Public Comment Period for the revised Children’s Dental Benefit are now posted to the Dental Listening Log.

Loan Repayment Directly Benefitting Colorado’s Safety Net
“Congratulations to the 26 recipients of the Kaiser Permanente Educational Loan Department for Safety Net Clinical Support Staff Program! CCHN received funding from Kaiser Permanente Community Health Fund to build upon workforce and clinical quality efforts over the next three years. CCHN developed the Kaiser Permanente Educational Loan Repayment for Safety Net Clinical Support Staff Program in partnership with Colorado CHCs, Colorado Rural Health Center (CRHC) and ClinicNET. In January 2014, CCHN, CRHC and ClinicNET kicked off the first loan repayment cycle of the grant period. Two additional loan repayment application cycles will occur in January 2015 and January 2016. During the first round of loan repayment, there was an outpouring of applications from the safety net organizations, receiving 113 eligible applications! Patient Navigation, Community Health Work and Care Coordination in Colorado Survey Results
“The Community Health Workers and Patient Navigator Work Group is a statewide coalition comprised of local and state agencies, advocacy groups, philanthropic organizations and public and private health plan representatives. Free, HRSA-Funded Nursing Leadership Course
“The Colorado Center for Nursing Excellence is in the third year of offering a HRSA-funded Nursing Leadership workshop. The final workshop dates under this grant are May 22-23 and May 28-30 (a total of five days in two segments), in Denver. This free workshop includes six months of weekly coaching by trained coaches. We have had 193 workshop participants since 2011, and they have given it very positive reviews. The focus is improving skills related to: change management; diversity and culture; effective communication; conflict resolution; negotiating and setting boundaries; and creating excellence in the workplace.
This HRSA-funded workshop was created to focus on Directors of Nursing and Assistant Directors of Nursing in the long term care sector, but the leadership development and coaching content is applicable to multiple positions and settings.

“CareMessage is a Stanford University-born award-winning 501(c)3 nonprofit health technology organization based in San Francisco. CareMessage helps clinics reduce no-shows and save time spent on patient education and outreach, while improving patient health outcomes and reducing overall costs. The technology complements existing clinic EMR systems and can be used by clinics without an EMR system.
With funding from the Packard Foundation CareMessage is distributing its service free of charge to Colorado's community safety net clinics for a year. Although this campaign is primarily targeted at free and charitable clinics, nearly all community safety net clinics serving the low-income residents of Colorado are eligible to participate.

On World Asthma Day May 6 and throughout the month, people with asthma and organizations dedicated to asthma control and education join together to increase awareness about asthma and improve the lives of all people with asthma. Informational webinar on Project A-L-L to improve the health of diabetes patients
“A-L-L is a systematic medication protocol that has shown to reduce the risk of heart attacks and strokes in patients with diabetes by 71% after five years of therapy. A-L-L stands for Aspirin, Lisinopril, and Lipid-Lowering Medication. It is a simple and evidence-based medication bundle that align with recent guideline recommendations and can be used as a population management tool for diabetes. Since 2006, A-L-L program has been successfully implemented in over 60 community health centers and hospital systems serving over 40,000 low-income, uninsured, medically underserved and vulnerable patients with diabetes in five regions (including Colorado).
The ultimate goal of Project A-L-L is to improve the health of diabetic patients, specifically by preventing cardiovascular events (heart attack and stroke). The Colorado Community Health Network (CCHN) recognizes that health care safety nets are essential partners in our shared goals of improving the health of individuals and communities. Connect for Health Colorado Updates
- “Connect for Health Colorado has signed up the 4th highest number of people for private health insurance among all states that operated their own Marketplaces, according to new data released Thursday from the Centers of Medicare and Medicaid Cervices (CMS). - The Colorado Division of Insurance announced Friday it will allow insurance companies to continue health plans for individuals and small groups that do not comply with the ACA’s minimum coverage for one more year, through 2015. Learn more.

“One in three U.S. adults has prediabetes; most are unaware they are at risk. CBS4 is continuing its prediabetes campaign to raise awareness through May 2014.
