We are interested in gathering information about non-federally qualified health center dental clinics and programs located across the county to address oral disease prevention, treatment and education needs of uninsured and underserved populations. These clinics and programs are a vital part of the dental infrastructure system for vulnerable populations.
We ask that you take 10-15 minutes to complete the survey. Only AACDP and Safety Net Solutions team members who will analyze survey data will have access to your clinic or program information. Safety Net Solutions will use information from the survey to help determine the need for and interest in technical assistance from non-FQHC dental clinics and programs.
