“The new National Safety Net Advancement Center (supported by a grant from the Robert Wood Johnson Foundation) is proud to issue a call for proposals (CFP) to facilitate safety net organizations in overcoming challenges posed by payment and care delivery reform. The goal of the CFP is to contribute to the growing evidence base on how to successfully implement payment and care delivery reforms in safety net organizations. Eligibility: 1) Federally Qualified Health Centers, 2) Community Mental Health Centers, 3) Public County Hospitals, 4) Tribal Health Centers, 5) Critical Access Hospitals, and 6) Safety Net Oral Heath Providers.
The CFP solicits proposals for two components of SNAC’s initiatives: 1) grant awards and 2) virtual learning collaboratives – applicants may choose to apply for either. Examples of potential funded grant projects are: clinical care team transformation strategies; clinical integration across settings; network structure, governance, and operations; risk management and adjustment strategies; financial planning, implementation, and control; and patient attribution and activation.
