ClinicNET Affiliates: 2014 Data Request and Annual Contribution Reminder!

ClinicNET affiliates should have received a 2014 Data Request in the mail a couple weeks ago. Please be sure to complete it and return it to us at 3033 S. Parker Road, Ste. 500, Aurora, CO 80014 or via fax to 720-863-7801.  If you prefer, you can fill out the electronic version and email it to johanna.gelderman@clinicnet.org.  We greatly appreciate you taking the time to complete this request so ClinicNET can most accurately advocate for you and represent your interests and concerns.
