“The AMA Foundation and AmeriCares have both released Requests for Proposals for two separate grant programs supporting safety net clinics.
Both grant programs will provide two-year grants of $10,000 to clinics to implement the CDC’s Diabetes Prevention Program (DPP). The goal of these programs is to increase the identification of prediabetes among clinic patients and improve their health-related outcomes.
Please note that while these programs are similar, they have separate application processes, implementation requirements and eligibility guidelines:
Interested clinics should carefully review both organizations’ programs to determine which opportunity is the best fit for them. Clinics will only be accepted into one of these two programs.
The deadline for submitting your application/ proposal to both programs is September 22, 2014. We encourage you to consider applying for one of these grant opportunities. Contact information is provided on each organization’s web site should you have any questions.” [Source:
