• The team organized the SIM plan into six “workstreams” to tackle the work ahead: (1) HIT and quality measures, (2) Payers and payment reform, (3) Data and evaluation, (4) Consumers and purchasers, (5) Provider and workforce, and (6) Service delivery and practice transformation.
