As a collaborating organization, ClinicNET is pleased to announce the official launch of the statewide Extension for Community Health Outcomes in Colorado (ECHO Colorado) program. ECHO Colorado is a statewide professional education initiative aimed at connecting health workforces to topic experts to increase access to specialty care and expert knowledge across underserved and rural areas using bi-directional video-conferencing technology.

Connect for Health Colorado Updates - DOI approves 2016 health insurance plans
- “The Colorado Division of Insurance announced approval of 2016 health insurance plans and rates. Increases in premiums average just under 10 percent for individual plans and a little more than 3 percent for small group. Of course, there are significant variations by area within the state. Read more.
- You can compare 2016 plans and premiums available to you, or someone you are helping, on our website right now. Because 2015 plans are still available for people who experience a Life Change Event, both plan years are available. Make sure you are looking under the correct effective date when you start shopping. Open Enrollment runs from November 1 through January 31.
- The Colorado HealthOP will not be offering coverage next year but individual members remain covered through the end of the year, as long as they continue paying premiums. Small business coverage ends on the renewal date. There have been media reports that some of the company’s network providers have declined to offer care to members. Colorado HealthOP provides guidance for that and other issues in an FAQ. Stakeholder Meetings Scheduled to Discuss Annualized Income Policy Proposals for Medicaid and CHP+
“The Department of Health Care Policy and Financing and Connect for Health Colorado are holding stakeholder meetings to discuss policy proposals to use annualized income for some Medicaid and CHP+ eligibility determinations. The next meeting will be held on Nov. 5 from 9:30 to 11:30 a.m. In-person attendance is strongly encouraged. During the meetings, HCPF and Connect for Health Colorado will:
- Provide an overview of annualized income policy proposals;
- Review the process and timeline for adoption of an annualized income policy; and
- Solicit stakeholder feedback on scenarios potentially impacted by this policy change.

ACC Phase II Stakeholder Meetings and NEW Concept Paper
“The Department of Health Care Policy and Financing (the Department) invites you to participate in guiding the Accountable Care Collaborative Phase II (ACC Phase II). The Overview and the full Concept Paper (just released) are meant to initiate conversations between the Department and the wide variety of stakeholders who deliver and receive services through Colorado Medicaid.
In order to facilitate dialogue and collaboration, the Department has scheduled a number of public meetings to discuss specific topics as they relate to ACC Phase II. Additional sessions will be announced at a later date that will explicitly address the areas of behavioral health and long-term services and supports. This program is for low income individuals and families who do not have access to any other coverage and meet program eligibility requirements. Colorado Bridge provides a Kaiser Permanente premium subsidy and medical financial assistance to support cost sharing, and enrollment in an off exchange Kaiser Permanente Individual and Family Plan. Provider billing workshops include both Colorado Medical Assistance Program billing instructions and a review of current billing procedures. Class descriptions and workshop calendars are also posted in the Provider Training section of the Medicaid website.
ICD-10 Updates from Medicaid:
External end-to-end testing has confirmed that Medicaid's systems are integrated, operable, and ready to accept the new ICD-10 codes and formats on October 1, 2015. Effective October 1, 2015, ICD-10 is now active and replacing ICD-9 diagnosis codes. Providers should note the ICD-10 implementation is Date of Service (DOS) specific:
- ICD-9 diagnosis and surgical procedure codes will be required on claims that contain DOS or dates of discharge (DOD) on or before September 30, 2015.
Claims submitted with both ICD-9 and ICD-10 codes will be rejected.
- ICD-10 diagnosis and surgical procedure codes are required on claims that contain DOS or DOD on or after October 1, 2015.
Medicaid billing manuals have been updated ICD-10 codes. Click here to access the manuals.

"EvidenceNOW Southwest is one of seven regional cooperatives funded by the Agency for Healthcare Research and Quality (AHRQ) to provide quality improvement services to small primary care practices, with 10 or fewer providers. Project ECHO Case Conferences: Identifying and Managing Patients at Risk for Cancer
"The opportunity is titled "Project ECHO Case Conferences: Identifying and Managing Patients at Risk for Cancer". "ECHO" stands for "Extension for Community Healthcare Outcomes". Lectures are free and participation is all online.
The Utah Cancer Control Program is hosting a regularly scheduled Project ECHO series for clinicians interested in learning more about identifying and managing patients at risk for cancer, with a specific emphasis on BRCA1/2 gene mutations and Lynch Syndrome. The series is hosted online twice a month at 12 pm Mountain Time, and participation is free. Each session includes a 10-20 minute lecture on various topics, and an opportunity for clinicians to consult with genetic counselors on real cases from their practice. Colorado Receives $11 Million Federal Grant for Transforming Clinical Practices Initiative (TCPI)
“The Centers for Medicare & Medicaid Services (CMS) has announced that Colorado's Collaborative for Practice Transformation is one of 39 health care collaborative networks selected to participate in the Transforming Clinical Practice Initiative. Colorado will receive up to $11 million over the next four years to provide technical assistance to help equip clinicians throughout Colorado with tools, information, and network support needed to improve quality of care, increase patients' access to information, and spend health care dollars more wisely. Health promotion is the process of supporting people to increase control over, and to improve, their health. It moves beyond a focus on individual behavior towards a wide range of social and environmental interventions. This series will provide information and case study support from national subject matter experts in health promotion.
Sessions will include: What is Health Promotion, Assessing Patient Readiness, Tracking Multiple Health Conditions, Resources for Health Promotion, Making Information Relevant, Using Self-Monitoring Tools, Dealing with Patient Perceptions.
ECHO Learning Community: The ECHO participants include the subject matter expert team (hub) and community healthcare workers (spokes). During ECHO sessions, a didactic is given by the subject matter expert team. Community healthcare workers then present cases using video conferencing equipment. Participants discuss the cases and recommendations are given. Grants to Free/Charitable Clinics to Establish CDC-recognized Lifestyle Change Programs for the Prevention of Type 2 Diabetes
“In collaboration with the American Medical Association’s Improving Health Outcomes department, the AMA Foundation will be awarding $10,000 grants to eight free clinics through the Healthy Communities/Healthy America program. Clinics will receive funding to implement the CDC’s National Diabetes Prevention Program over the course of two years. The 2015 Healthy Communities/Healthy America RFP is now available. All RFP’s must be submitted via email to amafoundation@ama-assn.org by November 6, 2015 at 5pm CST in order to be considered.
