Funding Opportunity: Advance Care Planning
As part of the Colorado Health Foundation's strategy to empower individuals and families to manage their own health, the Advance Care Planning funding opportunity will award grants of up to two years and up to approximately $300,000 for implementation of effective advance care planning initiatives in Colorado.
Advance care planning is important for health engagement, person-centered care, shared decision-making and achievement of Triple Aim objectives and helps ensure that health care wishes at end of life or in the event of incapability are honored.
This funding opportunity is designed to accelerate existing advance care planning efforts in Colorado where feasible and to support innovative demonstration projects that have the goal of increasing individual and family health engagement. Open for Applications: Strengthening the Safety Net: Capital Support
In 2009, the Colorado Health Foundation launched "Strengthening the Safety Net," which was intended to help Colorado's safety net clinics prepare for health reform. Since then, the Foundation has supported a wide array of clinics throughout the state including Federally Qualified Health Centers, rural health clinics, mental health centers and other community clinics for capital, operating and project grants. As primary care providers to underserved Coloradans, these clinics are important partners supporting our work to ensure all Coloradans achieve health with support from a network of primary health care and community services.
This capital funding opportunity is a continuation of the Strengthening the Safety Net initiative, and will support the expansion, construction or renovation of clinics to increase their capacity to provide team-based and patient-centered primary care to underserved Coloradans.
