With funding from the Packard Foundation CareMessage is distributing its service free of charge to Colorado's community safety net clinics for a year. Although this campaign is primarily targeted at free and charitable clinics, nearly all community safety net clinics serving the low-income residents of Colorado are eligible to participate.
Via the CareMessage dashboard, you can:
- Reduce No-Shows: Save time calling patients and reduce no-shows by using automated text and voice reminders. Manage all of your upcoming appointments and events by collecting RSVPs and recording attendance.
- Automate Follow-ups: Automate lab results, medication adherence, and tracking conditions (i.e. glucose monitoring, step count ,weight) so that you know how your patients are doing in between visits. Receive notifications alerting you of adverse changes in your patients’ health status.
- Rethink Disease Management: Improve patient education and disease management by utilizing our automated, evidence-based health coaching programs, including those designed to reach at-risk patients from minority populations.
- Monitor Status Changes: Keep clinic records up to date by automatically monitoring changes to phone numbers and addresses.
