Invitation to Participate as a Pilot Provider in The Life Quality Initiative

You are invited to participate in a grant-funded pilot of The Life Quality Initiative, intended to integrate palliative care principles and practices into the primary care setting. The goal is to learn what it takes to embed palliative care concepts into primary care earlier in the disease trajectory and within the trusted relationship of primary care.

The opportunity for interested clinics includes: provider and staff education on palliative care which includes advance care planning and symptom management; skills training for staff on how to have advance care planning with patients and families using the “Respecting Choices” methodology; and practice coaching to develop workflows, processes, documentation, etc. It starts with a focus population of about 30 patients to test the process. If it’s successful, the hope is that the practice will provide screening and services (or referral to a palliative care service).

The next CIVHC Connect is dedicated to our Health Care Delivery System Redesign efforts specific to Care Transitions and Palliative Care. Don't miss this opportunity to learn what's happening across Colorado to reach the Triple Aim through coordinated, patient-centered delivery systems.

The meeting will be offered twice for convenience - July 10th 2-4pm (COPIC Founders Board Room) and again on July 18th 2-4pm (The Colorado Trust Sabin Room).
