“The Women’s Wellness Connection Program is preparing to release two FY2015-16 Request For Applications (RFAs) for Care Coordination and Clinical Services (formerly known as Service Delivery) funds by the end of July 2015. These funds are for new contracts and were originally to be released through a Request for Proposals (RFP), but that has subsequently been changed to an RFA for both set of funds. Application documents and due dates for both RFAs will be available at www.colorado.gov/cdphe/wwc-funding. If you have any questions, please email them to the WWC Program at cdphe_wwc@state.co.us.
