Regional Health Connector Program Awards Contracts to Host Organizations

The Colorado Regional Health Connector (RHC) program is excited to announce that 11 new RHC Host Organizations have been selected to join seven existing RHC Host Organizations across Colorado. By the end of this year, 21 Host Organizations will be selected to support RHCs who will serve regions that cover the entire state.

"The Colorado Regional Health Connectors Program is soliciting proposals from local organizations throughout the state to host a Regional Health Connector.

The final Request for Proposals has been updated to include the newest version of Table 1: The Connection between Social Determinants of Health and Target Areas. We hope that this table is helpful to potential applicants as you contemplate the role of the Regional Health Connector in your community. The full RFP is available here.

Program staff will host weekly calls throughout June to respond to questions about the RFP:

Monday, June 20 11 a.m.-12 p.m.
Thursday, June 28 3:30 p.m.-4:30 p.m.
