**Like all US states, Colorado has a statewide cancer plan developed by cancer community members and supported by the Center for Disease Control and Prevention (CDC). These plans set forth measurable 5-year objectives with the potential to positively impact a state's cancer burden. Colorado's comprehensive 2016 - 2020 plan goals and objectives — currently under development — are intended to be a framework for collaborative efforts across the state that can empower individuals and organizations in Colorado’s fight against cancer. The questionnaire will take about 10 minutes to complete and the anonymous, aggregated results will be used to help CDPHE make decisions about how to move forward with implementing the plan. To complete the questionnaire, please click on the following link or cut and paste into your browser by May 5, 2015: https://www.surveymonkey.com/s/CancerPlan.” [Source: CDPHE]
