Community Transformation Grants call tomorrow!

The Centers for Disease Control and Prevention recently announced the availability of $100 million to support Community Transformation Grants, created by the Affordable Care Act (ACA). The funding is available to support evidence- and practice-based community and clinical prevention and wellness strategies that will lead to specific, measurable health outcomes to reduce chronic disease rates. CDC will be holding a conference call prior to the deadline for submitting Letters of Intent to apply for funding. The Letters of Intent are due to CDC on Monday, June 6, 2011. The call-in information is below:
