“The
SIM Practice RFA has been released and practices may begin submitting applications to be included in the first cohort of the
State Innovation Model (SIM). Practices that are selected to participate will receive technical assistance and financial support to integrate physical and behavioral health and test value-based payment models.

- September 15, 2015 - Practice RFA opens
- October 26, 2015 - Practice RFA closes
- December 25, 2015 - Selected practices notified
- February 1, 2016 - Cohort 1 practices kick-off
