Interested in SIM or EvidenceNOW Southwest? ClinicNET can be your Practice Transformation Organization!

ClinicNET has been selected as an approved Practice Transformation Organization (PTO) to provide practice facilitation services to practices participating in the Colorado State Innovation Model (SIM) and other potential initiatives.  ClinicNET also provides practice transformation coaching for EvidenceNOW Southwest.  We welcome the opportunity to work with you!

State Innovation Model (SIM):
The SIM Practice RFA is open and practices may begin submitting applications to be included in the first cohort of SIM. Applications are due October 26th! Practices that are selected to participate will receive technical assistance and financial support to integrate physical and behavioral health and test value-based payment models. EvidenceNOW Southwest (ENSW):
ENSW is aimed at transforming health care delivery by building critical infrastructure to help smaller primary care practices apply the latest medical research and tools to improve heart health. Practices will receive practice transformation and quality improvement support, including on-site practice facilitation and coaching, expert consultation, shared learning collaboratives, and electronic health record support. Applications are open!
