Colorado Health Access Fund - Proposals due May 15

The Denver Foundation released the first Request for Proposals (RFP) for the Colorado Health Access Fund. This statewide Fund supports programs and activities that increase access to behavioral health care and strives to improve behavioral health outcomes for people in Colorado with high health care needs. Over the next eight years (2015 – 2022), the Fund is committed to allocating resources among rural, urban, and suburban areas.

Organizations interested in applying for funding can identify the behavioral health need that they want to address in their community. Funding is available for projects, programs, capital improvements, collaboratives, and general operating support. The first round of applications is due May 15th; grants will be awarded in late July 2015. More details can be found at www.denverfoundation.org.

2015 Medicaid Fee Schedule

Colorado Cancer Plan Questionnaire

The Colorado Department of Public Health and Environment (CDPHE) invites you to complete a short questionnaire about your interest and perspective on the implementation of the 2016-2020 Colorado Cancer Plan.**
**Like all US states, Colorado has a statewide cancer plan developed by cancer community members and supported by the Center for Disease Control and Prevention (CDC). These plans set forth measurable 5-year objectives with the potential to positively impact a state's cancer burden. Colorado's comprehensive 2016 - 2020 plan goals and objectives — currently under development — are intended to be a framework for collaborative efforts across the state that can empower individuals and organizations in Colorado’s fight against cancer.

Denver Human Services Offers Navigator Training

Denver Human Services (DHS) is offering a navigator training for community partners. The navigator training includes an overview of the programs DHS offers, including food assistance, basic eligibility for Medicaid and cash programs, energy assistance, child support, childcare assistance program, PEAK, and a brief overview of the ACA. Remaining Non-ACA Compliant Health Insurance Plans Will End in 2015

The Colorado Division of Insurance (DOI) announced that all health insurance plans for individuals and small employers that do not meet Affordable Care Act (ACA) requirements will not continue into 2016 (note: this decision does not impact grandfathered plans, defined by the ACA as plans with an effective date prior to March 23, 2010). This means that insurance companies will not continue to offer these plans and consumers will not be able to renew these plans. For 2016, individuals and small employers that are not enrolled in grandfathered plans must enroll in ACA-compliant plans in order to receive coverage. Based on a survey of health insurance carriers the DOI conducted earlier this year and reflective of enrollment as of Dec. 31, 2014, 189,779 people will have their non-ACA compliant individual or small group plans discontinued by the end of 2015, and will have to choose a new ACA-compliant plan for 2016.

Insurance companies will notify all of their individuals and small employers who hold non-ACA compliant plans of the discontinuation. These notices must contain information about all available options; whether or not the company will offer plans that meet ACA requirements; finding ACA-compliant plans offered through Connect for Health Colorado; and the ability to purchase plans from other insurance companies. HCPF Updates: Enhanced rates for E&M and ACC Model Details and Policy Decisions

The Department of Health Care Policy and Financing (HCPF) is pleased to announce enhanced rates for E&M and vaccination codes are now loaded! Effective this week, any qualified E&M or vaccination claim will be paid at the December 2014 Medicare rate.  Providers are not required to attest to providing primary care. Our fiscal agent will also mass adjust all qualified claims paid since January 1st, and repay them at the higher rate.  Due to a limit on how many mass adjustments can be processed at a time, this process will take place over the next few weeks until all providers have been “made whole” for claims submitted since January 1st.

Please note that, although Medicare rates may be adjusted on a quarterly basis, Colorado Medicaid rates will not be adjusted to match these changes. HCPF will keep you posted if the rate benchmark changes.

ACC Model Details and Policy Decisions: Click here to view a document that summarizes three of the key structural decisions that will be in place starting July 1, 2017.  Over the next year, HCPF plans to continue stakeholder outreach and conduct additional research to continue developing the program model.

Parent and Caretaker Relative Eligibility Change FAQ Now Available: Beginning April 1, 2015, the income eligibility level for the Parent and Caretaker Relative MAGI Medicaid category was lowered from 107 percent of the Federal Poverty Level (FPL) to 68 percent FPL. Through a mass update in the Colorado Benefits Management System (CBMS), most parents and caretaker relatives who were enrolled in this category were administratively moved to the Adult MAGI Medicaid category (the category for adults ages 19 to 64 with FPLs up to 133 percent FPL). Parents and caretaker relatives with Medicare are not eligible for the Adult MAGI Medicaid category, but will likely be eligible for the Medicare Savings Program (MSP). A small number of people may not be eligible for MSP, but they will be eligible to apply for private insurance through a SEP through Connect for Health Colorado. HCPF created a Parent/Caretaker Relative Federal Poverty Level Changes Frequently Asked Questions (FAQ) document about this eligibility change.

Coloradans who meet the general requirements outlined below are strongly encouraged to submit an application through the Governor’s Office of Board and Commissions:
  • Board members must have knowledge of Medical Assistance programs;
  • Board members may include a person or persons who have received services through programs administered by HCPF;
  • No more than six members can be of the same political party;
  • At least one member shall be appointed from each congressional district;
  • Membership shall include representation by at least one member who is a person with a disability, a family member of a person with a disability, or a member of an advocacy group for persons with disabilities, provided that the other requirements are met. [Source: HCPF]

HHS announces new grant programs to help improve sharing of health information

“The Department of Health & Human Services recently announced the availability of $1 million in grant funds to support community projects for the Community Interoperability Health Information Exchange (HIE) Program. The funding will help support and enable the flow of health information at the community level, leading to better care and better health.

The Community Interoperability and HIE program will provide funds to up to ten community organizations, state or local government agencies, or other community groups. The awards will help unlock health information and better integrate community resources advancing better care and healthier communities.

Under the new grant program, awardees will demonstrate the use of health IT to the wide range of health providers, including those that are not eligible under the Medicare and Medicaid Electronic Health Record Incentive Programs. These include long-term and post-acute care providers, behavioral health providers, individuals and their caregivers, and other care setting and providers. These additional providers could also be safety net providers, public health, social service, emergency medical services and other key members of the care continuum.

Primary Care Fund application - Intent to Apply Notification due April 21st!

The Department has posted the 2015-16 Primary Care Fund application. The application includes important deadlines that must be met for consideration of the 2015-16 Primary Care Fund Grant including Intent to Apply Notification due April 21st and the actual application due by May 29th.

The Application looks a little different this year, in accordance with the American Disability Act; however there has not been any changes with regard to policies or requirements. In addition to the cosmetic changes to the application, the Department has created an Excel worksheet to assist Provider applicants with the Question 7 response. The worksheet is solely an option for providers to use. Both the application and Excel worksheet can be located in the Department’s website.” [Source: HCPF]

CCHAP Practice Manager Newsletter - Higher Medicaid Payments Approved for 2015

Medicaid 2015 fee schedule: CMS has approved the rate increase to E&M and vaccination codes, bringing them on par with Medicare rates published in December 2014. These rates took effect on January 1, 2015, but Medicaid was unable to pay the rates until CMS approval was received.  The rates are currently being loaded into the MMIS claims system. Once loaded, any qualified E&M or vaccination claim will be paid at the new rate. Providers are not required to attest to providing primary care. All qualified claims from January 1st, 2015 will be mass adjusted and repaid at the higher rate. Click here for the fee schedules.

National Correct Coding Initiative Challenges: The National Correct Coding Initiative (NCCI) was introduced to Medicaid claims processing for claims filed on and after October 1, 2010. The NCCI consists of code edit files and a manual that provides instruction on reporting multiple procedures and services or multiple units of a procedure, service, or supply. Practice Applications for BC3 Program are due by 5pm on April 17th

The BC3 Program is a 2 year initiative sponsored by The Colorado Health Foundation with practice transformation support provided HealthTeamWorks. The program areas of focus are: Quality Measures, Cost Reduction, Patient Engagement, Provider and Staff Experience, and Community Learning. The BC3 program intentionally aligns with other statewide programs such as the Comprehensive Primary Care Initiative and the State Innovation Model.

The practice requirements for participation in the BC3 Program are:
  • Previous practice transformation experience
  • QI Team established and meeting 2x/month
  • Practice Champion identified
  • Reporting Quality Measures in the HealthTeamWorks® Measures Database monthly
  • Completion of program evaluation surveys
  • Participation in community learning sessions and events

Practice Applications are due by 5pm on April 17th. Completed forms may be submitted: electronically to Heather Walker (hwalker@healthteamworks.org), by fax 303.934.6200, or by mail to the HealthTeamWorks® Office.  Outlined below the steps for the Practice Application:

There is limited space available in this program and practices will be selected for participation based on engagement and previous accomplishments in their transformation work. Practices will be notified if they have been selected to participate in the BC3 Program by April 30, 2015. Primary Care Fund application - Intent to Apply Notification due April 21st!

The Department has posted the 2015-16 Primary Care Fund applicationThe application includes important deadlines that must be met for consideration of the 2015-16 Primary Care Fund Grant including Intent to Apply Notification due April 21st and the actual application due by May 29th.

The Application looks a little different this year, in accordance with the American Disability Act; however there has not been any changes with regard to policies or requirements. In addition to the cosmetic changes to the application, the Department has created an Excel worksheet to assist Provider applicants with the Question 7 response. The worksheet is solely an option for providers to use. Both the application and Excel worksheet can be located in the Department’s website.” [Source: HCPF]

Safety Net Clinic Week (SNCW) will be here in no time!  The 6th annual SNCW will take place August 17-21.  In an effort to celebrate, support and raise awareness for Colorado’s health care safety net, the week is devoted to educating the public, policymakers and other community leaders about Colorado’s Community Safety Net Clinics (CSNCs) and federally certified Rural Health Clinics (RHCs). SIM Workgroup and Advisory Board – Application deadlines extended to April 17

The application deadline for SIM Workgroups has been extended to close of business on April 17th. You will find the links provided below to the application for the workgroups, criteria for each workgroup and the expectations for workgroup members. All applications should be submitted to gov_simgrant@state.co.us.
NOTE: Workgroup membership may preclude you or the organization you work for from competitively bidding on SIM grants.  Specifically if the workgroup members are significantly contributing to the design of work on working product that will be a part of the procurement opportunities that will become available later in the cooperative agreement.  The SIM office is working with the Attorney General's Office to clearly define what conflict of interest means when it comes to bidding on procurement. A position statement will be released once available. In addition, we have provided a diagram that will clarify the process for the workgroups.

The SIM Advisory Board application process has also been extended to Friday, April 17, 2015. Please submit your application no later than close of business on April 17th. Denver Foundation Request for Proposals: Colorado Health Access Fund

The Denver Foundation released the first Request for Proposals (RFP) for the Colorado Health Access Fund. This statewide Fund supports programs and activities that increase access to behavioral health care and strives to improve behavioral health outcomes for people in Colorado with high health care needs. Over the next eight years (2015 – 2022), the Fund is committed to allocating resources among rural, urban, and suburban areas.

The Denver Foundation commissioned the Colorado Health Institute (CHI) in 2014 to conduct an asset and gap analysis to glean a solid understanding of Colorado’s current diverse health care landscape to guide the development of this Request for Proposals. CHI’s report, which can be found on the Fund’s webpage, informed the Fund’s Advisory Committee and the decision to focus on behavioral health.

Organizations interested in applying for funding can identify the behavioral health need that they want to address in their community. Funding is available for projects, programs, capital improvements, collaboratives, and general operating support. The first round of applications is due May 15th; grants will be awarded in late July 2015. There are two optional opportunities for interested organizations to participate in Q&A sessions prior to the due date, and details can be found at www.denverfoundation.org.

Oral Health Colorado’s Industry News and Updates

Dental Trade Alliance Foundation: If your organization has an innovative project designed to improve oral health care for the growing number of needy Americans and would like to be considered for a grant, please review the guidelines. Each applicant must demonstrate that their innovative project will not only improve access to oral care in their immediate area, but also have the potential to grow and be implemented in other areas of the country.  Each applicant must complete the electronic 2015 DTA Foundation Grant Proposal Application Form. Grant applications are due no later than May 27, 2015.

DentaQuest Foundation Information Webinars: The DentaQuest Foundation is hosting a series of informational webinars on each of the Oral Health 2020 Targets over the next few weeks.  These target webinars will focus on regrounding us in the importance of each target, sharing a vision for how the target can be realized, hearing from the leaders in the field about what’s happening today, and wrapping up with exploring where we still need to go from here. Healthcare Policy and Finance Update: In case you missed the HCPF Colorado Medicaid Dental Program updates, here is a link for review. The revisions to the Adult Dental Services rule passed its final reading for permanent adoption at the Medical Services Board (MSB) meeting on March 13, 2015 and will be effective on April 30, 2015. 2015 National Health Service Corps Site Application Now Open!

The 2015 NHSC New Site application cycle is now open for eligible health care sites. The application cycle will close on June 2, 2015, at 7:30 p.m. ET.  Interested sites are encouraged to read the 2015 NHSC Site Reference Guide for all program requirements. Eligible sites include health care facilities that provide outpatient, ambulatory and primary health care services (medical, dental, and behavioral) to populations residing in high-need urban and rural areas.
