Colorado is preparing to apply for a competitive grant from the Center for Medicare & Medicaid Innovation (CMMI) within the U.S. Department of Health & Human Services. This funding opportunity is known as the State Innovation Models cooperative agreement or SIM Grant, and Colorado’s application is due to CMMI on September 17, 2012.
