Capitalize on Federal Dollars to Fund Practice Innovation

"Colorado practices have a unique opportunity to apply for two federal initiatives in the next two months to fuel practice transformation. The Colorado State Innovation Model (SIM), which helps integrate behavioral health and primary care in healthcare settings throughout the state, is preparing to recruit its second cohort of primary care practices. In addition, the Centers for Medicare & Medicaid Services (CMS) announced earlier today that Colorado was selected as one of 14 regions for the Comprehensive Primary Care Plus (CPC+) initiative, a new program that also seeks to transform care delivery in the state. The SIM Office is excited about this new opportunity for Colorado practices, and believes CPC+ dovetails with SIM efforts currently underway. The CPC+ practice application was released this month and more information on the application for the second cohort of SIM practices will be released by the end of the month. More information will be posted to the SIM website as it becomes available."
