“The Department has posted the 2015-16 Primary Care Fund application. The application includes important deadlines that must be met for consideration of the 2015-16 Primary Care Fund Grant including Intent to Apply Notification due April 21st and the actual application due by May 29th.
The Application looks a little different this year, in accordance with the American Disability Act; however there has not been any changes with regard to policies or requirements. In addition to the cosmetic changes to the application, the Department has created an Excel worksheet to assist Provider applicants with the Question 7 response. The worksheet is solely an option for providers to use. Both the application and Excel worksheet can be located in the Department’s website.” [Source: HCPF]
