The Colorado Medical Home Initiative invites all medical home stakeholders including you, your colleagues, families, policy makers, and organization/agency staff to attend the Medical Home Community Forum! The topic of this forum is Health Coverage Expansion & Enrollment under Health Reform. Click here for more information including the agenda, list of guest speakers, and location. If you are unable to attend, a conference call-in number and a live video (Livestream) event will be available.
