ClinicNET Affiliates will be receiving our 2015 Data Request in the mail soon. Please be sure to complete it and return it to us at 3033 S. Parker Road, Ste. 500, Aurora, CO 80014 or via fax to 720-863-7801.  If you prefer, you can fill out the electronic version and email it to johanna.gelderman@clinicnet.org.  We greatly appreciate you taking the time to complete this request so ClinicNET can most accurately advocate for you and represent your interests and concerns!
