As you know, the PEAK Mail Center was not available between July 9 and July 28, 2015. However, we would also like to inform you that this issue was tied to a breach that occurred between May 25 and July 5, 2015 in which client correspondence notices were mistakenly mailed to unintended recipients. It has been determined that a HIPAA breach impacted 1,622 CBMS clients and a social security number breach affected 1,258 CBMS clients. The technical error that caused notices to be mailed to unintended recipients was fixed on July 5 to prevent the problem from continuing.
