“The Women’s Wellness Connection Program has released it's Fiscal Year (FY) 2015-16 Request For Applications (RFA) for Care Coordination funds. Application documents and due dates can be found at www.colorado.gov/cdphe/wwc-funding. If your agency currently contracts with WWC for Care Coordination you do not need to reapply. If you have any questions, please email them to the WWC program staff at cdphe_wwc@state.co.us.

The WWC Program is preparing to release a FY2015-16 Request For Applications (RFA) for Clinical Services (formerly known as Service Delivery) funds by the end of July 2015. These funds are for new contracts and were originally to be released through a Request for Proposals (RFP), but that has now changed to an RFA.
