Apply Now for Amendment 35 Grant Funding

“The Amendment 35 Grants Program at the Colorado Department of Public Health and Environment is pleased to announce the coordinated released of the following Requests for Applications (RFA’s):

• The Cancer Cardiovascular Disease and Pulmonary Disease Competitive Grants Program
• Health Disparities Grant Program
• The Tobacco Education, Prevention and Cessation Grant Program

We have aligned each of the three application packages to look similar, and created digital application folders which include all the required documents for each RFA. Please note that agencies must apply individually for each grant program in accordance with the guidance outlined in each individual RFA.
