The Colorado Public Health Association (CPHA) is working hard to provide opportunities to share current information and projects from those doing patient navigation and community health work in Colorado. One such opportunity is the CPHA Annual Conference, September 19-20 in Breckenridge. Although the official deadline for abstract submission is closed, if you are interested in sharing your project information (lessons learned, processes, best/promising practices, successes, etc) regarding your work with patient navigators and community health workers, please send the following information to Andi Dwyer at andrea.dwyer.ucdenver.edu by COB Wed, May 29th.
